In accordance with the Professional Services Agreement entered into between
TEKsystems, Inc. ("Contractor") and the Cite of Fort Worth "Client" or "City") effective
6, 2013 and approved by M&C C-26551 +gin November 51, 2013 (the

"Agreement"), this Ap,pend,ix 12 is incorporated into the Professional Services
Agreement, and hereby reads as follows:
1 The term of this work order shall be extended to end when all services have
been completed under this work order as contemplated herein, but not later
than the end date set forth below.
2. This, Work Order incorporates by reference all of the terms and conditions of
the Professional Services Agreement, specifically the services to he provided
under Section 1 of said agreement.
3. The services to he performed under this Work Order shall be as fellows:
IT DATABASE ADMINISTRATOR - APPLICATIONS
1,. Day-to-day SQL Administration of the City's S,QL servers; includes correcting
issues with any of the SQL databases
2. Work with customers on questions and/or new updates to any
databases/reports
3. ''work with Application developers and venders in deploying and supporting
new databases, SSRS report requests, SS IS (integration services) packages
development and deployment, database migrations
4. Identify business re ,uirements, design elutions, and implement with the goal
of stand�ardizing processes, reuse of cede and ease of maintenance
5. Support Police and Fire Computer-Aided Dispatch database
6. Assist system administrator with user security, full--test indexing and disaster
recovery plans
7. Automate database maintenance tads such as index rebuilds, backups, and
leg shipping
8. Collect performance retries and data metrics for scalahility and capacity
,planning and code improvement suggestions
