WHEREAS, the City of Fort Worth (CITY) and Shield Engineering
Group, PLL , .. (ENGINEER) made and entered into City Secretary
Contract No. 4 3 8 6 2 (the CONTRACT) which was authorized by M&C C-
25817 on the� 11 day of 8eptembe�r�, 2012 in the amount of
$131, 291. 00; and
WHEREAS, the CONTRACT as sub�se,quently revised by: Amendment:
Number 1 in the amount of $ 8�, 3�5'9 . o 01 which was administratively
authorized on August 2113,, 2013 and Amendment No. 2 in, the amount of
$13, 068 ., 00 which was, administratively authorized on October 23,
2013; and
WHERFAS, the CONTRACT involves engineering servicels for the
following pirojIeict
Sanitary Sewer R.ehab i l i ta t i on, Contract, 8 .
NV Eu", it has become necessary to execute Amendment No. 3 to
the CONTRACT to include an increased, scope of work and revised fee
NOW THEREFORE, C TTY and ENGINEER, acting herein by and
through their duly authorized representatives, enter into the
following agreement, which amends the CONTRACT:

Article I of the CONTRACT is amended to 'include the additional
engineering services specified in a proposal dated January 22, 2014,r
a copy of which is attached hereto and incorporated herein. The cost
to City for the additional design services to be performed by
Engineer totals $43, 738. 00 (Water: $25, 742 . 00 Sewer: $17, 995'. 00) .

Article 1Z of the CONTRACT is amended to provide for an
increase in the fee to be paid to Engineer for as l work and sery ices
performed under the Contract,,, as amended, so that the total fee paid
by the City for all work and services shall be an amount of
$215,4616 . 00 .

Dear Soon,
Shield Engineering Group SEG is, to submit the following as amended services to the original
contract shorn above-.
61 Trench repair"rehabilitation plans and coordination for Eastwood rood Avenue $8,356
(approximately 979 LF). Trench repair was added to this project as requested sewer
by the c "mater Do t (Tony Sholola) due to recent citizen complaints and
observed failures. Effort includes initial' site visit, onsite -field meeting with city
officials from TPW and the Water Department, trench repair plans, field trips to
other trench repair locations to observe the effectiveness of other trench repair,
coord'i�nati!on with, TPW on the selected trench repair detail and geotelxtile materials.
Incorporate the 107 Temporary light of Entry exhibits intol the final plan construction $4,640
set (approximately 26 sheets). These sheets will provide the contractor with detailed (sewer)
information of the private sanitary s,eweer relocation effort on each of the lots. They
include coordinate locations of proposed cleanouta as well as removal limits of
driveways, sod, t roes, borings limits, etc, and items which will need to remain in place.
Prepare a stand alone plan set to replace and lower the existing waterline $25,742
along Hill'creat between camp Bowie and El Campo (approximately 3000 L . (water)
This effort includes preliminary opinion of probable costs and survey to locate the
existing meters, valves, etc. We anticipate that any necessary supplemental
information will be obtained from the existing plans along Hillcreat i.e. pavement
elevation, utility, crossings, etc) in order to limit design survey effort.
Additionall construction phase services anticipated for regular construction phase $5,000
eeltiings. This task includes weekly construction meetings, and coordination with sewer
contractor and residents for service lino re-alignments.
