SECOND AMENDMENT TO
CITY SECRETARY CONTRACT NO. 40678
ASBESTOS AND MOLD OPERATIONS 8C MAINTENANCE SERVICES
THIS SECOND AMENDMENT to City Secretary Contract. No. 40678 (hereinafter
"Amendment") is entered into by and between the City of Fort Worth ("City"), a home-
rule municipality in the State of Texas, acting by and through Fernando Costa, its duly
authorized Assistant City Manager, and Intercon Environmental. Inc., a Texas
corporation ("Contractor",), acting through Karen Andrews, its duly authorized President.
City and Contractor may be referred to herein individually as a Party, or eolleetively as
the Parties.
WHEREAS, the City and Contractor have entered into that certain agreement identified
as City Secretary Contract No. 40678 (also the "Contract") executed on August 11, 2010
for asbestos and mold operations and maintenance services; and
WHEREAS, that contract was far an initial term of one year beginning on August 11.
2010 through August l0, 2011:
WHEREAS, that contract had a provision under Article 4 (Term) for up to two (2)
extensions in the term of the contract for one(1) year each upon mutual agreement of the
parties;
WHEREAS, the parties in fact desire to extend the term for an additional year;
NOW WHEREFORE:
The City of Fort Worth and Intercon Environmental, Inc. do hereby agree to extend the
term of City Secretary Contract No. 40678 for one year beginning on August 11, 2012
through August 10, 2013.
Other than as specified in this amendment, no other changes are made to the terms and
conditions of the contract documents.
