(ENGINEER) made and entered into City Secretary Contract No. 35495, (the CONTRACT) which was authorized by M&C C-22179 on the 12th day of June, 2007 in the amount of $ 119, 297 . 00; and WHEREAS, the CONTRACT was subsequently revised by: Amendment No. 1 in the amount of $45, 250 . 00 by M&C C-22888 on July 8, 2008, Amendment No. 2 in the amount of $64 . 164 . 00 by M&C C-23543 on May 26, 2009, and Amendment No. 3 in the amount of $44, 385 . 00 which was administratively authorized on May 17, 2011; and WHEREAS, the CONTRACT involves engineering services for the following project : Storm Water Utility Project - Loving Avenue Channel and Culvert Improvements, Project Co. 00626; and WHEREAS, it has become necessary to execute Amendment No. 4 to the CONTRACT to include an increased scope of work and revised fee. NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1 . Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated March 13, 2014, a copy r of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals =` $30, 147. 00. (See Attached Funding Breakdown Sheet, Page -3-) 2 . M Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall be an amount of $303,243 . 00. (S&A)will provide professional surveying services on the above mentioned project. The scope of work of the land surveyor shall be to supply all supervision, technical labor, equipment, services, tools, consumables, insurance, and pay any applicable sales/use taxes, and all other things required to perform land surveying in accordance with technical requirements specified in this Estimate. • Project Name: Worth Hills Addition Replat. • Location of survey: Fort Worth, Texas(please see the attached exhibit for more details) • Purpose of Survey: The purpose of these services is to re-plat existing individual lots into consolidated lots and reflect proposed right-of-way changes on Prairie Avenue affecting each private lot. Project Scope: Plat Services Records Research: The Surveyor will research boundaries, subdivision plats, rights-of-way and easements of which the Surveyor has knowledge, which may affect the physical boundaries of the project. Easements with volume and page numbers will be identified and labeled on the survey submittal. Research shall include public record resources, including but not limited to the following: Fort Worth Records Vault Information and Storage Department. Tarrant County Records. Ownership or Easement Records as available. Deliverables Plat Services CAD Format: AutoCAD Civil3D 2011 format CAD files. Fort Worth Platting and comments: The project will strictly follow the guidelines for platting in Fort Worth, Texas. City of Fort Worth comments will be addressed promptly and submittals will be resubmitted within 5 business days of receipt of such comments. Project Scale: 1" = 20' or best fit for 24"x36" sheet(s). Base Map: An electronic base map at 1:1 scale (base unit US Survey Foot) containing all pertinent field data will be prepared. • Spooner and Associates, Inc. drafting standards including layers and styles will be used. • Text will be oriented with north being true reading West to East. • The base map shall be in AutoCAD DWG format. • High Quality NTCOG 6" orthos as a background. • 5 original executed black line hard copies of the Plat map are included. • 2 original executed mylars of the final plat are included in this estimate. Newly established lot lines and right-of-way: Newly established lots and right-of-way will be monumented in the field with appropriate survey monumentation. Fort Worth Council / Planning Meetings: This estimate does not include time associated with meeting with Fort Worth Planning department nor does it include time associated with City Council meetings. Meetings necessary for such items will be billed hourly ($115 per hour with mileage) at cost. Records Vault Research: This estimate does include City of Fort Worth records drawing research. Right of Entry: This estimate does not include right-of-entry services. Those services, if needed, will be serviced by the Engineer or Owner prior to the hereon described services. Work Schedule: 9 Topographic Survey: The surveyor will have the survey as described above completed within 4 weeks from the execution of project contract. DISCUSSION: The Amendment recommended by this Mayor and Council Communication will provide additional engineering services including re-platting of street right-of-way (ROW) for a permanent road closure, preparation of additional ROW/Easement documentation, and updated erosion protection design. The re- platting and easement effort is the result of a preferred design alternative involving a road closure that was developed during the course of design.
