Adoption Event Application PetSmart Charities and PetSmart stores have formed a partnership to help promote rnulti-agency adoption events at PetSmart stores. This is an application for approval to HOST an event at PetSmart. It is possible that an event will be approved but full or partial sponsorship funding may be unavailable. PART 1: Store Manager completes this page and submits no later than one month prior to event. EVENT DATES: STORE #: Event will be held: (circle one) in-store parking lot sidewalk grassy/median area COLLABORATIVE ADOPTION GOAL FOR EVENT: These events must be able to support a minimum goal of 40 adoptions; in order to achieve this goal,participating adoption partners must provide double the amount of pets available over the course of the event. NUMBER OF ADOPTION PARTNERS WILL BE PARTICIPATING IN THIS EVENT INCLUDE NAMES AND GROUP#'S OF PARTNERS PARTICIPATING IN THIS EVENT: All participants must be approved PetSmart Adoption Partners— Group Numbers must be visible in your POS STORE MANAGER RESPONSIBILITIES CHECK LIST: ❑ Evaluate and confirm that the selected space is capable of handling this size event (will not impede foot traffic, vehicular traffic, parking, etc.). Outdoor/Parking Lot Events may occur on premises outside of a store (sidewalk, grassy area or actual parking lot) dependent on landlord approval. Verify the lead organization has obtained any required local licenses or permits (i.e. City, Fire Marshal). Permit costs for must be included in the lead partner's budget for reimbursement. PetSmart Charities sponsorships for adoption events are limited, as we must spread the funding across many communities. If event is at a PetSmart store location, refrain from selling of products and/or fundraising. 5. Provide total adoption numbers after the event to PCI, broken out by species (dogs, cats. small pets) and participating organization. 6. If event is at a PetSmart store location, only approved PetSmart Charities partners can participate. 7. If the event is at an approved off-site venue the lead organization will receive a form for tracking adoptions that each participating organization must complete. As part of the event we've been invited to participate in, our organization agrees to: r. Greet and get to know pet parents—Your staff and volunteers love pets and so do our pet parents and associates. This is easy! 16 Treat everyone as potential adoptive pet parents—"I'm just looking" sometimes means "I'm shy." They may "just be looking" for someone else to refer to you! w Determine their needs—As you get to know them, you'll discover their image of the perfect pet. +it Educate, inform, and provide alternatives—You have the expertise that a potential adopter needs! Educate them on the adoption process, inform them of the responsibility of having a pet as well as their desired pet's history, provide alternatives if an adoption is not in the pet's and their best interest. i.
