WHEREAS, the City of Fort Worth (City) and Teague Nall and
Perkins, (Engineer) made and entered into City Secretary Contract
No. 27966, (the Contract) which was authorized by the City Council
by M&C C-19206 on the 27th day of August, 2002 and subsequently
revised by one previous amendment; and
WHEREAS, the Contract involves engineering services for the
following project:
Sanitary Sewer Rehabilitation Contract LII (52)
WHEREAS, it has become necessary to execute Amendment No. 2 to
said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through
their duly authorized representatives, enter into the following
agreement which amends the Contract:

Article I of the Contract is amended to include the additional
engineering services specified in a proposal letter dated September
27, 2005, a copy of which is attached hereto and incorporated
herein. The cost to City for the additional design services to be
performed by Engineer total $4,100.00.

Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not
exceed the sum of $316,150.00.
3.
All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effect.

Fee Proposal for Contract Amendment No. 2
Dear Mr. Chaubey:
Teague Nall and Perkins, Inc. is submitting this fee proposal for Amendment No. 2 to the
contract for the above referenced project based on additional services requested. The
following is a brief description and cost for the additional services provided on this project:
1. Additional services to re-design sewer lateral L-1981
The alignment of the sewer must be revised to accommodate a new strip shopping
center. The additional services include design and preparation of plans.
Subtotal Re-design of Sewer Lateral L-1981: $2,000
2. Additional Services to prepare Right-of-Entry and Easement Documents
Two easement parcels must be revised as a result of the new alignment of the
sewer. The rates used to calculate the appropriate compensation are the same as
what was used in the original contract and are specified below:
• Permanent Utility Easements 2 each @ $700 per each = $ 1,400
• Temporary Construction Esmts. 2 each @ $350 per each = $ 700
Subtotal Preparation of Right-of-Entry and Easement Documents: $2,100

For additional services provided pursuant to this Amendment No. 2, we request
compensation in an amount not to exceed $4,100.00. Such payment shall be made in
accordance with the payment terms specified in the Contract.
Summary of Contract Fee:
Original Contract Amount: $290,300
Amendment No. 1 (Not-to-Exceed): $ 21,750
Amendment No. 2 (Not-to-Exceed): 4,100
Revised Contract Amount (Not-to-Exceed): $316,150
