SECOND AMENDMENT TO CITY SECRETARY CONTRACT NO. 39329
PROFESSIONAL SERVICES AGREEMENT

This Second Amendment to City Secretary Contract No. 39329 — Professional Services Agreement ("First
Amendment") is made and entered into this ',_754L, day of September, 2014, by and between the City of
Fort Worth ("CITY"), a home-rule municipal corporation of the State of Texas, acting by and through its
duly authorized Assistant City Manager, and Occupational Health Centers of the Southwest, P.A., DBA
Concentra Medical Centers ("CONCENTRA"), a Texas Professional Association, acting by and through
its duly authorized Senior Vice President and Chief Medical Officer.
WITNESSETH:
WHEREAS, CITY and CONCENTRA entered into a Professional Services Agreement effective
October 1, 2009, the same being Fort Worth City Secretary Contract No. 39329 (the "Agreement"), and
WHEREAS, pursuant to the Agreement, CONCENTRA provides occupational injury and non-injury
occupational health care services to CITY employees; and
WHEREAS, in 2011, the parties amended the Agreement to revise service locations and provide
for additional immunizations, that amendment being Fort Worth City Secretary Contract No. 39829-A1; and
WHEREAS, the Agreement provides for an initial, three-year term and two, one-year renewal
options, with the final renewal expiring on September 30, 2014; and
WHEREAS, on August 12, 2014 the Fort Worth City Council authorized the City Manager to
execute an extension of the Agreement through December 31, 2014; and
WHEREAS, CITY and CONCENTRA wish to extend the Agreement, as amended, from October 1,
2014 through December 31, 2014.
NOW THEREFORE, in consideration of the foregoing and the mutual covenants and agreements
contained herein, CITY and CONCENTRA hereby agree as follows:

1. Fort Worth City Secretary Contract No. 39329, as amended, is hereby further amended to revise
Section 2, "Agreement Term"to add a new Section 2.3 to read as follows:
2.3 On expiration of the second renewal term under Section 2.2, CONCENTRA and CITY
mutually agree to renew and extend this Agreement (Fort Worth City Secretary Contract
No. 39329), as amended by Fort Worth City Secretary Contract No. 39329-A1, for a three-
month Extension Period, beginning October 1, 2014 and ending December 31, 2014,
under the same terms and conditions unless otherwise further amended herein.
2. All other terms and conditions of Fort Worth City Secretary Contract No. 39329, as amended by
Fort Worth City Secretary Contract 39329-A1, that are not amended herein shall remain in full force and
effect throughout the Extension Period.
