Home My WebLink AboutContract 30417 AMENDMENT NO. 6 STATE OF TEXAS $ CITY SECRETARY CONTRACT NO.3 I] (No M&C Needed) COUN'T'Y OF TARRANT $ WHEREAS, the City of Fort Worth (City) and LOPEZGARCIA GROUP formerly Wendy Lopez & Associates, Inc . , (Engineer) made and entered into City Secretary Contract No. 26994, (the Contract) which was authorized by the City Council by M&C C-18701 on the 9th day of August 2001; and subsequently revised by five previous amendments; and WHEREAS, the Contract involves engineering services for the following project: Engineering contract for the design of Water and Sanitary Sewer Extension for Bryson Tract Area. WHEREAS, it has become necessary to execute Amendment No. 6 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated July 13, 2004, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $15,554 . 08 . 2. Article II of the Contract is amended to provide for an increase in the maximum fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of $132,519.26 . Project Manager City of Fort Worth Department of Engineering 900 Monroe Street, Suite 201 Fort Worth, Texas 76102 RE: Request for Amendment No. 6 Water and Sewer Extension for Bryson Tract, D.O.E. 3312 LGGROUP Project No. 210069.00 Dear Mr. Domenech: This letter is in response to your request for additional engineering design and special services for the above referenced project. The additional design services are as described below. A detailed breakdown of the proposed fees for these additional services is also attached. Our proposed engineering fee for the additional work includes the following (also identified in Attachment E-1): 1. Design 680 LF of 8" waterline along Bailey-Boswell Road from Bryson Lane to Old Decatur Road, including two (2) 8" gate valves, required domestic connections, and a single fire hydrant. This additional design includes a total revision to Waterline "C" (Sheet 12) and a revision to 8"Waterline "A" at Bailey-Boswell Road (Sheet 4). 2. Change the proposed 6" water service line (Waterline "B", Sheet 12) from Old Decatur Road to Tract 1C (James M. Dean property) to a 4" water service (Sheet 12). 3. Add a 6" fire line service for the Bryson Elementary School on 8" Waterline "A" in Bryson Lane (Sheet 4). 4. Redesign sanitary sewer Line "A" by providing a manhole and stub-out at approximately station 19+88. This design will connect sanitary sewer Line "A"to the sanitary sewer system to be built by the developer of the Boswell Meadows Addition. This requires a total revision to Sheet 7. Sheets 5 and 6 showing the current alignment for sanitary sewer Line "A" will be revised to indicate "Removed from this Contract". These two sheets will remain in the existing plans. 5. Prepare a plan sheet identifying the vacation of existing sanitary sewer and utility easements. 6. Perform a sewerage subbasin capacity study and verify the sizing of the sanitary sewers designed by the Boswell Meadows Addition Developer's engineer. I ; . Attend one (1) conceptual meeting, (2) Eagle Mountain-Saginaw Independent School District design meetings, and one (1) Preliminary Engineering Meeting for a total meeting time of 12 hours. 8. Prepare and submit a written statement that verifies the Bryson Tract sanitary sewerage system improvements can be constructed (dry) prior to the Boswell Meadows Addition sanitary sewerage system improvements. 9. Revise the existing index and layout control sheets (Sheets 2 & 3) to reflect the Amendment No. 6 revisions. 10. Prepare required opinions of probable cost which identify expected changes in construction cost. 11. Make miscellaneous note changes (references) to the plans to accommodate the newest design changes. 12. Prepare a traffic control sheet and provide the required details to accommodate the sanitary sewer construction in Old Decatur Road. 13. Prepare one (1) private service re-connection sheet for 6, 4"services. Please note that the proposed fee does not include the following work items: ✓ Preparation of easements or property descriptions, ✓ Preparation of Temporary Right-of-Entry Exhibits and Documentation, ✓ Bid assistance, ✓ Submission of anytype of electronic file, except as specifically indicated, ✓ Preparation of permits for various local, state, or federal authorities, ✓ Environmental documents or wetland determinations, ✓ Geotechnical studies or subsurface investigations, ✓ Storm Water Pollution Prevention Plan, ✓ Additional meetings, site visits, or reviews, except as specifically indicated, ✓ Construction layout surveying, or ✓ Other items not specifically identified within this document. The additional efforts are considered outside our original scope of work, and we therefore request a contract amendment to increase the lump sum fee amount by $15,554.08 (See attached proposed LGGROUP fee).
