COUNTY OF TARRANT S
WHEREAS, the City of Fort Worth (City) and Teague Nall and
Perkins, Inc. , (Engineer) made and entered into City Secretary
Contract No. 27917, (the Contract) which was authorized by the City
Council by M&C C-19183; on the 8th day of August 2002; and
WHEREAS, the Contract involves engineering services for the
following project:
Water Replacement Contract 2002 STM-C, F and G
WHEREAS, it has become necessary to execute Amendment No. 1 to
said Contract to include an increased scope of work and revised
maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through
their duly authorized representatives, enter into the following
agreement which amends the Contract:

Article I of the Contract is amended to include the additional
engineering services specified in a proposal letter dated September
27, 2004 , a copy of which is attached hereto and incorporated
herein. The cost to City for the additional design services to be
performed by Engineer total $11,238.00.

Article II of the Contract is amended to provide for an
increase in the maximum fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall not
exceed the sum of $199,454 . 00.

All other provisions of the Contract which are not expressly amended
herein shall remain in full force and effec(t�.

Fee Proposal for Contract Amendment No. 1
Dear Mr. Sholola:
Teague Nall and Perkins, Inc. is submitting this fee proposal for Amendment No. 1 to the contract
for the above referenced project based on additional services requested. The following is a brief
description and cost for the additional services provided on this project:
1. Additional Services to Subdivide the Project into Multiple Construction Contracts
The original engineering services contract was for the preparation of one set of construction
plans and specifications for the entire project. The additional compensation for preparing
plans and specifications for multiple projects is summarized below:
• Basic service for preparing a cover sheet, project
layout sheet, and survey control sheet per project,
STM-F and STM-G = $ 2,710
• Additional compensation for printing plans and
specifications for each project, STM—F and STM—G = $ 5,528
Subtotal Subdividing Project into Multiple Constr. Contracts = $ 8,238
2. Additional Service to Prepare an SWPPP for General Construction Permit
The engineering services contractdoes not include the preparation of an SW3P forgeneral
construction permitting. The contract was negotiated before the new regulations of the
Texas Commission on Environmental Quality(TCEQ)required that an SW3P be prepared
for construction activities that result in the disturbance of one to five acres. The total length
of water line to be constructed in STM—F is 9,532 LF. Considering a 5 foot trench width for
the water line replacement down each street,the total disturbed area is 1.09 acres. As for
the STM-G project, the total disturbed area is less than one acre and therefore does not
require an SW31P be prepared.

For additional services provided pursuant to this Amendment No. 1,we request compensation in an
amount not to exceed $11,238.00. Such payment shall be made in accordance with the payment
terms specified in the Contract.

Amendment No. 1 (Not-to-Exceed): $ 11,238.00
Revised Contract Amount (Not-to-Exceed): $199,454.00
