• Additional street light system and traffic control during negotiations with Town of Westove41
Hills for work on Westover Drive.
• Mobilization and demobilization expenses involved with returning a komatsu 650 excavator to Q
the job site for the 20 foot deep sewer installation on Westover Drive. The excavator had to be
moved to a San Antonio project during the seven month time frame it took to reach an
agreement with the Town of Westover Hills for work on Westover Drive. U

This proposal includes the transportation cost for mobilization and demobilization to the project. This includes bringing the 650
excavator from an active project in Round Rock and returning it to that location. This specific machine is needed at minimized
clearance between the road surface and the the tree canopy. The schedule was pushed to respond to additional aborist preparation
and several Westover Hills Council meetings and as a result the machine had to be pulled off a working project near Austin to
respond to the availability of the work zone&special handling because of the minimal tree canopy clearance above the road
surface. Portions of this work include a22 VF cut below road surface insignificantly competent limestone.
Once SJL receives written acceptance of this CMR-8 Dated 9/4/14 the work can be scheduled so that the equipment can be
onsite as early as mid October to the first of November 2014.

Description of Work
This proposal is to address the transportation cost for Arborilogical to meet on site with the City of Fort Worth,and Ed Barry
(Bass Representative)to discuss the tree canopy on the Bass property.
The City of Fort Worth Water Engineering Department has directed S.J.Louis to submit a proposal for a professional aborist
firm,Arborilogical Services,Inc.,to prepare a cost estimate. This CUR when approved will cover the cost to have the above
done.
The Arborist is to schedule a site visit through S.J.Louis,City of Fort Worth,and the Bass group Yepresentative sometime this
week.
Arborilogical Services,Inc.quote will include the following:
The cost to provide a a video and still photos taken with a graphic representation of how the tree canopy will look if the trees are
actually trimmed with Arborilogical Services,Inc.marking and supervision to provide a clear and straight up above and between
both curbs on Westover Drive from the intersection of Deepdale and Westover Drive at Station 57+97 to some 600 Linear Feet
up hill to Station 64+05.

The City of Fort Worth Water Engineering Department has directed S.J.Louis to submit a proposal for Fowlkes.Norman to meet with the
residehts an Westover Drive&to quote the earingltrimming of the tree canopy. This includes quotes dated 4h 0114 for the following street
addresses:

SJL has had an ongoing additional Temporary Street Lights:tystern&Special Liner Plate Shaft Shoring System on Westover Drive sitting idle
while the CFW Stop Work Notice required for this portion of the work& clo the.City of Westover.Hills evaluates if overhead tree canopy trim
and clearing is permissible. Both the Temporary Street.Light System&Special Liner Plate-Shaft shoring have been idle since November 2013.
SJL requires written acknowledgement of an In House Change order of this proposal&.the direction to proceed.
The In House Change Order must include this work and the most previous other-Arbodst work from Arbodogigical Services.
