AMENDMENT No. 3 COMTRACT I D.
TO CITY SECRETARY CONTRACT
WHEREAS, the City of Fort Worth (CITY) and Kimley-Horn and
Associates, Inc. , (ENGINEER) made and entered into City Secretary
Contract No. 45237, (the CONTRACT) which was authorized by M&C C-25254 on
the 9th day of November, 2011 in the amount of $ 61, 138 . 00; and
WHEREAS, the CONTRACT involves engineering services for the
following project :
Ridglea Urban Village Streetscape Improvements; and
WHEREAS, the CONTRACT was subsequently revised by: Amendment Number
1 in the amount of $1, 300 . 00 which was administratively authorized on
June 6, 2012, Amendment Number 2 in the amount of $21, 000. 00 which was
administratively authorized on September 3, 2013; and
WHEREAS, it has become necessary to execute Amendment No. 3 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:
1.
Article I of the CONTRACT is amended to include the additional
engineering services specified in a proposal dated September 5, 2014, a
copy of which is attached hereto and incorporated herein. The cost to
City for the additional design services to be performed by Engineer
totals $5, 513. 00.

Article II of the CONTRACT is amended to provide for an increase in
C) the fee to be paid to Engineer for all work and services performed under
CD the Contract, as amended, so that the total fee paid by the City for all
work and services shall be an amount of $88, 951. 00.

All other provisions of the Contract, which are not expressly
amended herein, shall remain in full force and effect.

Ridglea Urban Village Streetscape Improvements
Dear Patrina:
Kimley-Horn and Associates, Inc. ("Kimley-Horn" or "Consultant") is pleased to submit this
Amendment#3 to the City of Fort Worth (City) to provide additional services to update the plans and
project manual for re-bidding.
The following Scope of Services has been developed based on our discussions with the City.
Scope of Services
Kimley-Horn will provide the services specifically set forth below.
Task 1 —Updating Plans, Estimates, Schedule, and Project Manual
Kimley-Horn will perform the following tasks:
(A) Update plan layout sheets to clearly indicate whether the work is part of the base bid or the
additive alternate bids.
(B) Update quantity summary sheet to show the quantities broken out by the new base bid and
additive alternate bids.
(C) Update the following TxDOT documents:
(i) Construction schedule (break out by base bid and additive alternates)
(ii) Summary Estimates
(iii) Opinion of Probable Construction Cost
(D) Project Manual updates including:
(i) 00 11 13 Invitation to Bidders
(ii) 00 41 00 Bid Form
(iii) 00 42 43 Proposal Form
Task 2 —Bidding Phase Services
Kimley-Horn will perform the following tasks:

(A) Attend Pre-Bid conference.
(B) Address Contractor questions and issue addenda as necessary
(C) Attend Bid Opening
(D) Bid Analysis and Tabulation
(E) Preparation of four(4) copies of the Project Manual for execution.
Additional Services
Any services not specifically provided for in the above scope will be billed as additional services and
performed at our then current hourly rates
Schedule
We will provide our services in a mutually agreed upon schedule following Notice to Proceed from the
City. Additional services, if desired, will be performed in a timely manner upon authorization by the
City.
Fee and Expenses
Kimley-Horn will perform the services in Tasks 1 and 2 for a total lump sum fee of $5,513. All
permitting, application, and similar project fees will be paid directly by the Client.
Lump sum fees will be invoiced monthly based upon the overall percentage of services performed.
