understand that the provision of water by the City of Fort Worth shall be made in strict compliance with Chapter 35 of
the Fort Worth City Code(1986), as amended, specifically but not limited to the following Sections:
1.City Code Section 35-60"Uses Outside City" as applicable;and
2.City Code Section 35-20"Provisions Incorporated into Consumer Contracts"as applicable;and
"All of the provisions of this Chapter[35]shall be deemed to be incorporated in every contract between the city
and its water or sanitary sewerage service customers,and each customer or consumer shall be charged with
knowledge of the provisions of this chapter and, by applying for and accepting water or sanitary sewerage
service from the city,to have assented to the provisions of this chapter."This shall include any and all
ordinances adopted and amending Chapter 35 such as ARTICLE VI. LAWN AND LANDSCAPE IRRIGATION
CONSERVATION.
3. City Code Section 35,Article III"Charges"as applicable; and
The owner understands that he/she is responsible for any related impact fees, tap fees, meter deposits,front foot
charge,per acre charge,water main capacity charge and extension under CFA or miscellaneous project. Please
obtain the amount to these charges from the Fort Worth Water Development Section.
4.City Code Section 26-2 A 01.2 as applicable; and
The owner needs to apply for a plumbing permit with the Fort Worth Planning& Development Department to
connect to the City of Fort Worth water and/or sanitary sewer lines despite the property is in the county.
5. City Code Section 12.5.Article V"Public Drinking Water"and Chapter 35,Article VII "Installation& Maintenance
of Irrigation System"as applicable;and
The owner is responsible for the installation of backflow devices, obtaining permit through the Fort Worth Planning
and Development Department and test the devices annually according to this section of the city code for both
domestic and irrigation systems.

6. Local Government Code Sec.212.012. "Connection of Utilities, Platting Requirement" as applicable.
An entity may not serve or connect any land with water,sewer,electricity,gas,or other utility service unless the entity
has been presented with or otherwise holds a certificate applicable to the land issued under Section 212.0115.
Under this section the property is required to be platted in order to receive service.
I understand that by making this application, I am entering into a contractual agreement with the City of Fort Worth and
hereby agree to the terms and conditions stated above. I certify that I am authorized to make this application and to
enter into such agreement.

HEARING:
SUBJECT: Authorize Provision of Water and Sanitary Sewer Service at 1.25 Times the Retail Rate to
Serve Property Located at the Southwest Corner of Avondale Haslet Road and Haslet
Court, in Tarrant County, Pursuant to City Code Section 35-60B, Uses Outside City
(Adjacent to COUNCIL DISTRICT 7)
RECOMMENDATION:
It is recommended that the City Council:
1. Authorize the Water Department to provide water and sanitary sewer service at 1.25 times the
retail rate to property located at the southwest corner of Avondale Haslet Road and Haslet Court in
Tarrant County, pursuant to City Code Section 35-6013; and
2. Authorize the execution of an Agreement for such services.
DISCUSSION:
Mr. Matthew Mcleod, the owner of the property located at the southwest corner of Avondale Haslet
Road and Haslet Court has submitted a request to receive water and sanitary sewer service from the
City of Fort Worth. The legal description of the property is: Lots 1 and 2, Block 1 of Avondale Haslet
MOB Addition, located in the extraterritorial jurisdiction of Fort Worth, inside Tarrant County and
within the Fort Worth Water Certificate of Convenience and Necessity (CCN). This property is
scheduled for annexation at the end of this year, but the applicant has requested to receive water and
sewer services at this time.
Mr. Matthew Mcleod has agreed to pay all fees and charges associated with the connection. The
applicant also understands that the fees and the monthly bill will be 1.25 times the rate for customers
inside the City of Fort Worth.
This property is located adjacent to COUNCIL DISTRICT 7, Mapsco 4V.
FISCAL INFORMATION/CERTIFICATION:
The Financial Management Services Director certifies that the Water Department is responsible for
the collection and deposit of funds due to the City.
