FIRST AMENDMENT TO CITY SECRETARY
CONTRACT NO. 45065
WHEREAS, on October 31, 2013,the City of Fort Worth("City") Jackey R. Dunn Jr.
d/b/a Romance Services, a sole proprietorship authorized to do business in the State of Texas
("Contractor)made and entered into City Secretary Contract No. 45065 (the"Contract) for
construction services for the City's Priority Repair Program.
WHEREAS,the Priority Repair Program provides up to $5,000.00 for home repairs for
eligible City homeowners for Priority 1 emergency repairs and Priority 2 repairs of failed
household mechanical systems to address health and safety issues as more particularly defined
by the program.
WHEREAS, the Contract was for a 1 year term beginning on the Effective Date and
provided for 4 additional 1 year terms;
WHEREAS, the Parties have agreed to extend the Contract for an additional 1 year term
beginning November 1,2014 through October 31, 2015; and,
WHEREAS, it is the mutual desire of City and Contractor to amend the Contract to meet
Contract objectives.
NOW, THEREFORE, City and Contractor hereby agree to amend the Contract as
follows:
I.
ARTICLE 1. DEFINITIONS is amended by adding the following definitions:
"Effective Date"means November 1, 2014.
"Priority Repair Program" or"program"means the City's home repair program for
eligible low to moderate income homeowners adopted by the City Council in M&C G-
17589.
ARTICLE 2. SCOPE OF SERVICES Section A. is amended to be and read as follows

"A. The purpose of this Contract is to provide home repair services for the Priority
rri Repair Program. The services provided shall be either Priority 1 emergency
repairs or Priority 2 repairs of failed household mechanical systems to address
health and safety issues as determined by City in accordance with program

The first sentence of ARTICLE 5. TERM OF CONTRACT is amended to be and
read as follows:
"This Contract shall commence as of the Effective Date and shall automatically expire
upon the earlier of the expenditure of$300,000.00 or October 31, 2015 (the "Expiration
Date"), if not terminated sooner or extended as described below."
II.
Attachment C attached to the Contract is deleted and replaced with the attached
Attachment C Revised 10/2014.
III.
All other terms and conditions of City Secretary Contract No. 45065 not amended herein
remain unaffected and in full force and effect, are binding upon the Parties, and are hereby
ratified by the Parties. Capitalized terms not defined herein shall have meanings assigned to
them in the Contract.

PLUMBING SERVICES: All services shall be completed in accordance to local code
requirements. "Contractor is responsible for securing permit (when applicable). All items
shall operate without leakage, noise, vibration or hammering. All penetrations of building
components hall be neat and sleeved where applicable. No solder containing lead shall be
used in any pipe or fixture damage to structural members from drilling or notching shall be
repaired.
1. Product or Service Type: GAS LINE: — Install new CSST corrugated Stainless Steel
tubing (Gastite's Flash Shield or PRO-FLEX) installation shall include all new shut off
valves and any labor or materials necessary to install and replace the existing lines
under the house. Contractor to apply and pay for all permits.

3. HOURLY RATE FOR LABOR MASTER PLUMBER: Services will be provided Monday
through Friday between 7:00 am and 5:30 pm. Rate shall include trip charge, travel
time, mileage, transportation cost, parking and toll fees. Rate shall start upon arrival at
client home or worksite. "Contractor is responsible for securing permit, (when
applicable).

14. DRAIN-WASTE-VENT-PVC: Install schedule 40 PVC pipe and fittings, solvent welded
after a dyed cleaning step. Install pipe with hangers Ton center without critical damage
to structural members.

15.SEWER SERVICE-4" PVC: Install a 4" PVC sewer line from structure and connect to the
public sanitary sewer system. Install clean-outs as required. Re-grade yard in work area,
sow grass seed and spread straw to reestablish lawn. Contractor to apply and pay for
all permits.

16. GAS LINE—PRESSURE TEST: Cap all gas lines prior to filling the distribution system
with 5-8 lbs. of compressed air. Maintain pressure for at least 30 minutes. Locate and
seal minor any leaks in the system.

17.GAS LINE–SCHEDULE 40 STEEL: Hang 3/4" main and 1/2"fixture gas supply lines of
schedule 40 black steel pipe with malleable iron screwed fittings. Install brass stop
valves at all equipment.

WATER HEATERS: Installation cost shall include all travel time,trip charge, labor&
equipment/material/supplies required to install complete new water heater. Install a glass
lined, high recovery, insulated, gas water heater with a 6-year warranty (electric water heater
when applicable). Use A. 0. Smith or approved equal. Installation cost shall include new
double wall vent pipes collard new gas line, "drip leg" new T&P lines, new roof flue/lack stack-
complete, pan and pipe insulation. Unit must meet city code and must be raised 18" from
elevation (when applicable). The removal of existing water heater and proper disposal shall be
in accordance to EPA regulation. "Contractor is responsible for securing permit."
18.WATER HEATER-30 GALLON GAS:

valve inside structure. Install PEX type 3/4" fixture grouping trunk/manifold, and 3/8"
individual supply lines to service two 3 piece bath, water heater, and kitchen and
laundry area. Follow Manufacture Installation Guide using all PEX fittings, and crimp
rings, shall be complete, code compliant,fully usable, and include shut-off valves,
chromed fixture supply lines and air gaps. Insulate all pipes where applicable to include
over the attic areas.

with removable material such as "Thumb Gum"to keep out any rodents. Also attach
