City of Fort Worth Department of Engineering 1000 Throckmorton Fort Worth, TX 76101 Re: Supplemental Authorization Request No. 1 Morningside Neighborhood Drainage Improvements Project 00088 DOE 4737 Mr. Argomaniz: Based on our February 15, 2005 meeting, it was determined that the alignment of the proposed 72" storm drain along Mississippi Avenue would be located east of the existing concrete pavement. Topographic survey for the project was completed prior to the referenced meeting and was based on the conceptual plan from August 2002, covering only the limits within the ROW. To accommodate an alignment outside of the pavement, it will be necessary to acquire additional survey information. This alignment will also require the preparation and acquisition of easements not originally anticipated. This letter is a request for supplemental authorization to provide additional surveying and easement preparation to accommodate this change. 1. Additional Topographic Survey TCB will provide the additional survey required to accommodate the proposed storm drain location. 30 feet of additional survey information will be obtained east of the Mississippi ROW for an approximate distance of 1000 If. Our fee to provide this additional survey is $3,000 (1000 if at $3 per If). 2. Additional Permanent Easements It is anticipated that the proposed alignment will require the preparation of up to five (5) permanent easements. Currently, our contract allows for two (2). Three (3) additional easements will be prepared in concurrence with current City standards. Our fee to prepare these additional easements is $1,800 (3 easements at$600 each). Items 1 and 2 will be performed by our subconsultant, Gorr*dona & Associates, Inc. Their proposal for the additional work is attached. l Established in 1946 Engineering Excellence for Over One-Half Century TurnerCollieOBradcn Inc. Mr. Richard Argomaniz, P.E.
