COUNTY OF TARRANT §
WHEREAS, the City of Fort Worth (City) and Huitt-Zollars, Inc (Engineer) made
and entered into City Secretary Contract No. 29546 (the Contract) which was authorized
by the City Council by M&C C-19958 on the 2nd day of February, 2004; and
WHEREAS, the Contract involves engineering services for the following project:
Engineering Service for Reconstruction of Pavement on Donnelly Avenue
(Alamo Street to Ashland Street)
WHEREAS, it has become necessary to execute Amendment No. 1 to said
Contract to include an increased scope of work and revised maximum fee;
NOW THEREFORE, City and Engineer, acting herein by and through their duly
authorized representatives, enter into the following agreement, which amends the
Contract:
1.
Article I of the Contract is amended to include the additional engineering services
specified in a proposal letter dated May 20, 2004, a copy of which is attached hereto and
incorporated herein. The cost to City for the additional services to be performed by
Engineer total $2,000.00
2.
Article H of the Contract is amended to provide for an increase in the maximum
fee to be paid to Engineer for all work and services performed under the Contract, as
amended, so that the total fee paid by the City for all work and services shall not exceed
the sum of$43,637.00
3.
All other provisions of the contract, which are not expressly amended herein, shall
remain in full force and effect.

Additional Services for Coordination of Utilities
Dear Mr. Sahu:
Huitt-Zollars, Inc. (HZI) has prepared the paving plans for the above-referenced project. The water and
sewer replacement plans have been prepared by Teague, Nall and Perkins, Inc. At your request, HZI will
combine the paving, utility plans and specifications into a single set of construction documents. Outlined
below is the additional cost incurred by HZI to combine the plans and specifications and create a single
cover sheet that will have professional engineer seals from both companies.
SUMMARY OF ADDITIONAL REQUIREMENTS
• Create Single Cover for Plans
• Revise Specifications to include Proposal for Utilities and Pertinent Specification Section, as well
as a Bid Summary.
• Clerical for Specifications
• Additional Project Coordination
COMPENSATION
• HZI will complete the additional work required for the additional fee of$2,000.
