TO CITY SECRETARY CONTRACT
WHEREAS, the City of Fort Worth (CITY) and
Visintainer Engineering, PLLC (ENGINEER) made and entered into
City Secretary Contract No. 45925, (the CONTRACT) which was
authorized by M&C C-26933, on the 26th day of August, 2014 in
the amount of $260,428. 00; and
WHEREAS, the CONTRACT was subsequently revised by
Amendment Number 1 in the amount $35,780 . 00 which was
administratively authorized on the 9th day of October, 2014;
and
WHEREAS, the CONTRACT involves engineering services for the
following project:
Water and Sanitary Sewer Replacement Contract 2014, WSM-C
WHEREAS, it has become necessary to execute Amendment No. 2
to the CONTRACT to include an increased scope of work and
revised fee
NOW THEREFORE, CITY and ENGINEER, acting herein by and
through their duly authorized representatives, enter into the
following agreement, which amends the CONTRACT:
1 .
Article I of the CONTRACT is amended to include the additional
engineering services specified in a proposal dated December
15, 2014 , a copy of which is attached hereto and incorporated
herein. The cost to City for the additional design services to
be performed by Engineer totals $39,808 . 00 .
2 .
Article II of the CONTRACT is amended to provide for an
increase in the fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall be
an amount of $336,016 . 00 .

All other provisions of the Contract, which are not expressly
amended herein, shall remain in full force and effect.

Visintainer Engineering, PLLC is pleased to submit the following as amended services to the original
contract, CPN number shown above:
Additional effort includes survey, design, bid phase services, construction phase services and
reproduction for additional or extended water and sanitary sewer lines on Rockhiil Road, including 1500
LF of 8-inch water line and 1332 LF of 8-inch sanitary sewer line.
Amendment 2
Survey $3,000.00
Design and Bid/Construction Phase Services $35,600.00
Reproduction $50.00
-10%Mgmt $1.158.00
$39,808.00
This amendment does not include easement preparation for the extension of the Donna Street water line
as the easement requirements are currently under review by Tarrant County Transportation Group.
The total change/addendum for design survey would be$35,780.
Original contract $260,428.00

$336,016.00
The new contract amount will be$336,016.00. The original M/WBE requirement was 10%. The original
contract participation was 33%. With Amendment No. 2,the M/WBE participation is at 32.75%.
