(No M&C Needed) COUNTY OF TARRANT $ WHEREAS, the City of Fort Worth (City) and Halff Associates Inc. , (Engineer) made and entered into City Secretary Contract No. 23668, (the Contract) which was authorized by the City Council by M&C C-16627 on the 24th day of February, 1998; and subsequently revised by five previous amendments; and WHEREAS, the Contract involves engineering services for the following project: Engineering contract for the design of Main C1C4B Drainage Area Sanitary Sewer System Rehabilitation and Improvements (Group 6, Contract 2) , Part 4. WHEREAS, it has become necessary to execute Amendment No. 6 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated March 14, 2000, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $5,200.00. 2 . Article II of the Contract is amended to provide for an increase in the maximum fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of $354,465.00 . City of Fort Worth Department of Engineering 1000 Throckmorton Fort Worth, Texas 76102 Re: Amendment No. 6 Main C 1 C413 Drainage Area Sanitary Sewer System Rehabilitation and Improvements (Group 6, Contract 2), Part 4 DOE No. 1829 Dear Mr. Domenech: In response to your request for additional modifications following numerous Final Reviews to the Main C 1 C4B Part 9 and Part 10 plans and specifications, Halff Associates is pleased to present this Amendment for the following additional services: Main C1C4B, Part 9 and Part 10 • Update Part 9 plans to include City general comments as noted and discussed during our telephone conversation of March 7, 2000, on Plan set dated 1/13/2000. Note, all previous comments from previous City reviews were addressed in the current plan set. • Coordinate changes to Plans and Specifications with Wade & Associates regarding latest comments on Part 9. • Realign 640 linear feet of Part 10 plans (25% of project) to relocate the proposed M- C 1 C4 in Fournier Street. • Update plans to include City general comments as noted and discussed during our meeting of February 17, 2000, on Plan set dated 12/22/99. Douglas Rademaker, Director, DOE and Mike Groomer, Assistant City Manager FROM: Mike Domenech, DOE DATE: March 20, 2000 RE: DESIGN FEE AMENDMENT NO. 6 TO MAIN C1C4B DRAINAGE AREA SANITARY SEWER REHABILITATION AND IMPROVE- MENTS (GROUP 6, CONTRACT 2), PART 4, DOE NO. 1829 The proposed amendment will reimburse Halff Associates, Inc., for the additional engi- neering services that will be provided on the project as requested by the City. At the direction of City staff Halff provided modifications to the original design. The modifications include realignment of a major portion of the sanitary sewer line and the revision of the proposal section of specifications. Halff Associates, Inc., has requested a fee of$5,200.00 for the additional services to be provided thereby increasing the contract fee from $349,265.00 to $354,465.00. I believe this fee is fair and reasonable for the additional services that were provided. Please authorize this additional design fee by your signatures on the attached contract amendment form.
