AMENDMENT NO. 2 TO
CITY SECRETARY CONTRACT NO. 45518
PROFESSIONAL SERVICES AGREEMENT
This Amendment is entered into by and between the City of Fort Worth (hereafter
"City"), a home rule municipal corporation organized under the laws of the State of Texas, with
its principal place of business at 1000 Throckmorton Street, Fort Worth, Texas, and Net
Transcripts, Inc. (hereafter"Vendor" or"Net Transcripts" or"NT").
WHEREAS, the parties have previously entered into City of Fort Worth City Secretary
Contract No.45518 (the"Contract"), which became effective on April 7, 2014; and

w WHEREAS, the Contract involves the Vendor providing and performing audio
transcription services; and
WHEREAS, the original term of the Contract expired on September 30, 2014; and
WHEREAS, the parties entered into City of Fort Worth Contract No. 45518-A1
("Renewal") to renew the term of the Contract until September 30, 2015. The Renewal was
executed on December 23, 2014.
WHEREAS, the City and the Vendor now wish to amend the original Contract to
increase the maximum dollar amount allowed for the compensation of services.

NOW, THEREFORE, City and the Vendor, acting herein by the through their duly
authorized representatives, enter into the following agreement to amend the contract:

j Section 3.0 COMPENSATION FOR SERVICES shall now be amended to state "the
maximum dollar amount shall not exceed$20,000 annually and no additional compensation shall
i be paid for any other expenses incurred unless first approved by City."
i 2.
All other provisions of the Contract which are not expressly amended herein shall remain
in full force and effect.
