AMENDMENT No.3 CONTRACT NO. 4?AXI-JU
TO CITY SECRETARY CONTRACT
WHEREAS, the City of Fort Worth (CITY) and Kimley-Horn
and Associates, Inc. , (ENGINEER) made and entered into City
Secretary Contract No. 43427, (the CONTRACT) which was
authorized by M&C C-25728 on the 17th day of July, 2012 in the
amount of $453, 700. 00; and
WHEREAS, the CONTRACT was subsequently revised by:
Amendment No. 1 in the amount of $320,500 .00 which was
authorized by M&C C-26223 on May 7, 2013 and Amendment No. 2
in the amount of $138, 600.00 which was authorized by
M&C C-27241 on March 24, 2015; and
WHERF.A19, the CONTRACT involves engineering services for
the following project:
Old Decatur Road f McLeroy Boulevard to Longhorn Road;
and
WHEREAS, it has become necessary to execute Amendment
No. 3 to the CONTRACT to include an increased scope of work
and revised fee.
NOWTHEREFORE, CITY and ENGINEER, acting herein by and
through their duly authorized representatives, enter into the
following agreement, which amends the CONTRACT:

Article I of the CONTRACT is amended to include the
additional engineering services specified in a proposal dated
April 29, 2015, a copy of which is attached hereto and
incorporated herein. The cost to City for the additional

design services to be performed by Engineer totals $32 ,745.00 .

o Article II of the CONTRACT is amended to provide for an
3 increase in the fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall be
an amount of $945,545. 00 .

All other provisions of the Contract, which are not
expressly amended herein, shall remain in full force and
effect.

Dear Wilma:
Kimley-Horn and Associates, Inc. has attached the scope of services for Amendment 3 to the City of
Fort Worth to provide additional design services for adding 2,000 linear feet of water line design and
1,350 linear feet of sanitary sewer design to the Old Decatur Road—McLeroy Boulevard to Longhorn
Road Project.
The Consultant will perform the Services described in Attachment A on a labor fee basis. The
recommended breakdown for water and sewer is as follows:
