AMENDMENT No. 2 RACT NO° 45�a�f
TO CITY SECRETARY CONTRACT
WHEREAS, the City of Fort Worth (CITY) and Shield Engineering
Group, PLLC. (ENGINEER) made and entered into City Secretary
Contract No.45924, (the CONTRACT) which was authorized by M&C C-
26932 on the 26th day of August, 2014 in the amount of
$244,534 .00; and
WHEREAS, the CONTRACT was subsequently revised by: Amendment
Number 1 in the amount $135,352 . 00 authorized on the 18th day of
March, 2015 M&C C-27190; and
WHEREAS, the CONTRACT involves engineering services for the
following project:
Water Sanitary Sewer Replacement Contract 2014, WSM - A; and
WHEREAS, it has become necessary to execute Amendment No.2 to
the CONTRACT to include an increased scope of work and revised
fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and
through their duly authorized representatives, enter into the
n following agreement, which amends the CONTRACT:

3 Article I of the CONTRACT is amended to include the
-a� additional engineering services specified in a proposal dated
I" May 11, 2015 a copy of which is attached hereto and incorporated
o herein. The cost to City for the additional design services to be
v' performed by Engineer totals $25, 616. 00 .

Article II of the CONTRACT is amended to provide for an
increase in the fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall be an
amount of $405,502 . 00 .

Re: Water and Sanitary Sewer Replacements Contract 2014 WSM-A: CPN 02369
Dear Robert,
Shield Engineering Group (SEG) is pleased to submit the following as amended services to the original
contract shown above:
• Prepare Sanitary Sewer Plan and Profiles to replace existing lines M-6, $11,940
L-644and L-5363 (approximately 1020 LF). Plans include sewer (SS Part 2 Yr 2 CIP)
details, overall sewer map exhibit, and existing sewer service connections.
Topographic survey services for design is included in this task item.
• Revise plan sets to move Sylvania, Retta, Ackers, Blevins, Colonial $2,760
and First sewer and water design plans from the Part 2 Year 2 CIP (W&SS Part 3 Non-CIP)
plan submittal into the Part 3 Non-CIP plan submittal.
(approximately 35 sheets)
• Provide approximately 8 hours of efforts with the Transportation $1,020
and Public Works Group for the incorporation of specifications, (TPW Part 2 & Part 3)
details and bid proposal provided by TPW into the Part 2 and
Part 3 bid books. This scope assumes that TPW will be signing and
sealing any necessary details and specs.
• Provide Bid Phase Services for Part 2 and Part 3 construction plans. $4,025
Both bid services include a pre-bid meeting at the City with the (W&SS Part 2 Yr 2 CIP)
Contractors. $4,025

• Additional Reproduction Costs for Part 2 and Part 3 construction plans. $750
