IT IS AGREED that the Contract effective January 1, 2015 between DELTA DENTAL INSURANCE
COMPANY and CITY OF FORT WORTH is hereby AMENDED effective January 1,2015 as follows:
Section 8.03 in ARTICLE 8 TERMINATION AND RENEWAL is DELETED and REPLACED as follows:
8.03 if Contractholder notifies Delta Dental that it intends to terminate this Contract upon less than 30 days
notice,Section 8.02 will apply as if Delta Dental terminated this Contract under Section 8.01 second bullet,
provided,however, that if the Contractholder requests termination of this Contract due to a lack of
appropriation by its governing body, no payment shall be required other than payments due through the
effective date of termination.
Except as AMENDED all terms and provisions of this Contract shall remain unchanged.
