AMENDMENT AND RENEWAL NO. 1 TO
CITY SECRETARY CONTRACT NO. 46395
PROFESSIONAL SERVICES AGREEMENT
This Amendment and Renewal is entered into by and between the City of Fort Worth
(hereafter "City"), a home rule municipal corporation situated in portions of Tarrant, Denton and
Wise Counties, Texas, with its principal place of business at 1000 Throckrnorton Street, Fort
Worth, Texas, and Hulen Hills Animal Hospital (hereafter "Consultant"), a Texas Corporation
acting by and through Kristen Thomasen, its duly authorized Director of Corporate
Development, each individually referred to as a "party" and collectively referred to as the
"parties."
WHEREAS, the parties have previously entered into City of Fort Worth City Secretary
Contract No. 46395 (the "Contract") for the purpose of veterinary services for the Police Department
K-9 Unit, which was executed on January 30, 2015 with an effective date of December 15, 2014,
and
WHEREAS,the City Council authorized three (3) one (1) year renewal options; and
WHEREAS, the original term of the Contract expires on December 14, 2015; and
WHEREAS, the City and Hulen Hills Animal Hospital wish to amend the Contract to
incorporate the available renewals; and
WHEREAS, the City and Hulen Hills Animal Hospital now wish to exercise the first
renewal option to extend the term of the Contract through December 14, 2016.
NOW, THEREFORE, City and Consultant, acting herein by and through their duly
authorized representatives, enter into the following agreement to amend the contract:
1.
Section 2 Term.
The term of the Contract is hereby amended to extend the term to expire December 14, 2016,
unless terminated earlier in accordance with the provisions of the Contract. Extension of the
term shall not require nor allow an increase of Consultant's Fee under the Contract.
After execution of this first renewal, the Contract has two (2) additional one-year options to
renew.

2.
All other provisions of the Contract which are not expressly amended herein shall remain
in full force and effect.
