AMENDMENT NO. 1 TO
CITY SECRETARY CONTRACT NO. 4j1 J4
This Amendment is entered into by and between the City of Fort Worth ("City"), a
hone rule municipal corporation situated in portions of Tarrant, Denton, Parker and Wise
Counties, Texas and Monica Azzolini, DV ("Contractor"), a Texas licensed veterinarian, each
individually referred to as a"party"and collectively referred to as the "parties".
WHEREAS, the parties have previously entered into City of Fort Worth City Secretary
Contract No. 47124 (the "Contract") for veterinary services, which was executed on October 2,
2015; and
WHEREAS, the Contract had a provision for renewal on a month-to-month basis; and
WHEREAS, the parties now contemplate a longer term arrangement than originally
anticipated, and executing a new amendment every month is an unnecessary administrative
burden which simply increases costs and uncertainty for both parties; and
WHEREAS, the parties wish to extend the term of the agreement by six months; and
WHEREAS, the parties may still terminate this agreement in accordance with the terms
of the Contract; and
NOW, THEREFORE, City and Contractor, acting herein by the through their duly
authorized representatives, enter into the following agreement to amend the Contract:
1.
The City and Contractor agree to extend City Secretary Contract No. 47124 to extend the
term from November 1, 2015 to May 1, 2016. The Contract, as amended, may be terminated in
accordance with Section 4 of the Contract or by execution of a superseding contract between the
parties.
2.
All other provisions of the Contract which are not expressly amended herein shall remain
unchanged and in full force and effect. The not-to-exceed amount of the Contract is expressly
not increased by this amendment.
