WHW Transport, Inc d/b/a Cornish Wrecker Service
Contract No. 45017
Renewal Term: October 1, 2015 — September 30, 2016
The above-referenced Agreement, which expired on September 30, 2014, has four renewal options. The
first renewal option expired on September 30, 2015. This letter is to inform you that the City requests
renewal of CSC No. 45017 for an additional one year period, which will begin October 1, 2015. All
other terms and conditions of CSC No. 45017 remain unchanged. If you agree to extend the existing
contract for an additional 12-month period, please mark the appropriate box and sign below.
Please return your signed renewal letter, along with a copy of your current insurance certificate.
