Contract No. 45452 (the"Agreement")
Renewal Term: October 1, 2015—September 30, 2016
The above-referenced Agreement, which expired on September 30, 2015, has four renewal options. Pursuant to
the Agreement, renewals are at the City of Fort Worth's ("City") option. This letter is to inform you that the City
requests renewal of Contract No. 45452 for an additional one year period, which will begin immediately and will end
on September 30, 2016. All other terms and conditions of Contract No. #45452 remain unchanged. Please return
your signed agreement letter, along with a copy of your current insurance certificate, to the address set
forth below.
