In accordance with the Professional Services Agreement entered into between
TEKsystems, Inc. ("Contractor") and the City of Fort Worth ("Client" or "City") effective
as of November 6, 2013 and approved by M&C C-26551 on November 5, 2013 (the
"Agreement"), this Appendix B-35A is incorporated into the Professional Services
Agreement and hereby reads as follows-
1. The term of this work order shall be extended to end when all services have
been completed under this work order as contemplated herein, but not later
than the end date set forth below.
2. This Work Order incorporates by reference all of the terms and conditions of
the Professional Services Agreement, specifically the services to be provided
under Section 1 of said agreement.
3. The services to be performed under this Work Order shall be as follows:
IT PC Support Technician
a. Migrate computer systems from Windows XP to Windows 7 according to
requirements.
b. Ensure successful imaging, application installation and user state migration for
each personal computer (PC) assigned.
C. Ensure computer peripherals are installed and working correctly or identified as
non-functional.
d. Answer general Windows 7 functionality questions from respective PC user.
e. Provide technical assistance to PC users in a courteous and professional
manner during deployment.
f. Effectively escalate risk or quality issues.

4. Personnel assigned to provide services under this Work Order:

End Date: September 3, 2016
Payment Terms:
Payment Rate: $39.95 per hour
5. This Appendix B-35A shall terminate immediately in the event the contract
personnel does not begin to provide contract services to the City under this
Appendix as of the Start Date stated above. If the contract personnel begins to
provide services, but Contractor provides notice to the City that the contract
personnel will no longer provide services under the Appendix, and such notice
is effective prior to the End Date stated above, then this Appendix shall
terminate immediately as of the effective date of the notice from Contractor.
