' AN ORDINANCE INCREASING THE ESTIMATED RECEIPTS IN THE GENERAL FUND, FUND O1, '~ MISCELLANEOUS REVENUE, OF THE CITY OF FORT WORTH FOR THE FISCAL YEAR 1988-89 BY TRANSFERRING THE SUM OF $171,340 FROM THE CAPITAL PROJECTS RESERVE FUND, FUND 10, ACCOUNT NO 010001-00, UNSPECIFIED PROJECTS, TO THE GENERAL FUND, AND APPROPRIATING SAID $171,340 TO THE GENERAL FUND, FUND O1, FIRE DEPARTMENT, TRAINING DIVISION, ACCOUNT NO 36- 80-00, SAID FUNDS TO BE USED FOR THE PURCHASE OF AUTOMATIC EXTERNAL DEFIBRILLATORS, PROVIDING FOR A SEVERABILITY CLAUSE, MAKING THIS ORDINANCE CUMULATIVE OF PRIOR ORDINANCES AND REPEALING ALL PRIOR ORDINANCES IN CONFLICT HEREWITH, PROVIDING FOR ENGROSSMENT AND ENROLLMENT; AND PROVIDING AN EFFECTIVE DATE. BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF FORT WORTH, TEXAS: SECTION 1 That in addition to those amounts allocated to the various City departments for the Fiscal Year 1988-89 and in the Budget of the City Manager, there shall also be increased estimated receipts in the General Fund, Fund O1, Miscellaneous Revenue, by transferring the sum of $171,340 from the Capital Projects Reserve Fund, Fund 10, Account No 010001-00, Unspecified Projects, and appropriating said $171,340 to the General Fund, Fund O1, Fire Department, Training Division, Account No 36-80-00, said funds to be used for the purchase of automatic external defibrillators SECTION 2. Should any portion, section or part of a section of this ordinance be declared invalid, inoperative or void for any reason by a court of competent jurisdiction, such decision, opinion or judgement shall in no way impair the remaining portions, sections, or parts of sections of this ordinance, which said remaining provisions shall be and remain in full force and effect. SECTION 3 That this ordinance shall be cumulative of Ordinance No 10151 and all other ordinances and appropriations amending the same except in those instances where the provisions of this ordinance are in direct conflict with such other ordinances and appropriations, in which instance said conflicting provisions of said prior ordinances and appropriations are hereby expressly repealed SECTION 4. The City Secretary of the City of Fort Worth is hereby directed to engross and enroll this ordinance by copying the caption and effective date into the minutes of the City Council and by filing this ordinance in the ordinance records of the City of Fort Worth. SECTION 5. This ordinance shall take effect and be in full force and effect from and after the date of its passage, and it is so ordained. Adopt the attached supplemental appropriations ordinance transferring $171,340 from the Capital Projects Reserve Fund 10, Project No. 010001-00, Unspecified Projects, to the General Fund O1, and appropriating this amount in General Fund O1, Fire Department Training Division, Account No. 36-80-00, and 2. Authorize $171,340 to be used for the purchase of automatic external defibrillators. DISCUSSION• Functions of Defibrillators The application of an electrical shock to patients experiencing certain types of abnormal heart rhythms often results in the conversion of the lethal rhythm to a normal rhythm. As a result, many patients experiencing heart attacks survive to live full and healthy lives. The less time that expires between the attack and the application of such a shock, the greater the chances of successful conversion of the heart rhythm. Medical literature finds that every twenty-second improvement in time to defibrillation increases the patient's chances of survival by one percent. An external defibrillator is the device used by medical personnel to deliver such electrical shocks to heart attack victims. Eighty percent of such conditions result in fibrillation of the heart rhythm. When applied to patients presenting such conditions, the defibrillator can be successful i.n "converting" the abnormal rhythm to a normal rhythm. Until a few years ago, the external defibrillator was only available in the hospital setting, or to the paramedic-level emergency medical technician. Advances in technology resulted in the development of the automatic external defibrillator (AE D), a device which prompts the user through visual and/or audio cues as to the appropriateness of use and application. Because of these advances, it is not necessary for the user to 'be trained in the recognition of the abnormal rhythms that may benefit from defibrillation. The machine is programmed to monitor the patient's heart rhythm and recognize the lethal rhythm that may be safely converted to normal through defibrillation. The user then applies self adhesive pads that deliver the shock, presses, th.e;7app,ropriate buttons on the machine, and stands away from the patient. ~~:~ -~p.r,~e ~~,~~~~~ The AED then delivers a pre-programmed level of shock, and again monitors the patient's heart to determine if the lethal rhythm has been converted. Recent studies of the use of AE D's published in th.e New England Journal. of Medicine found that of 276 patients wh.o were initially treated by firefighters using AED's, 84 (30 percent) survived to hospital discharge compared with 44 (19 percent) of 228 patients when firefighters delivered only basic cardiopulmonary resuscitation and the first defibrillation was performed after the arrival of the paramedic ambulance crew. The Dallas Fire Department reports that 24 additional lives 'were saved during a six month period through the use of automatic external defibrillators. Fort Worth Fire Department Defibrillator Program In 1986, during the implementation of the original MedStar system, both an AED program for the Fire Department and an automatic vehicle location system for ambulances was proposed as system upgrades. It was decided at that time not to pursue such upgrades because of the complexities of transitioning from the then-current EMS system to the MedStar system. In April, 1987, the Fort Worth Ambulance Authority approved the implementation of these upgrades. The ETAK automatic vehicle location system has been installed and is operating successfully throughout the MedStar service area. Also in April, 1987, the Fort Worth Fire Department Emergency Medical Services (EMS) office began evaluating AED's far use by the Fire Department EMS first responders. The Emergency Physicians Advisory Board and Dr. John Gwin, MedStar Medical Director, have recommended the acquisition and use of AED's by the Fire Department a.s an upgrade to Fort Worth's emergency medical services system. The Fire Department responds to an average of about 250 cardiac calls per month, and arrives on the scene of 200 (80~) of these calls prior to the MedStar unit. The number of Fort Worth residents who will survive heart attacks of the type that can be treated with the defibrillator will increase as a result of the early arrival of Fire Department personnel equipped with AED's. A companion M&C recommends the purchase of sufficient AE b's to allow the' equipping of each Fire Department EMS responding vehicle with a unit. Although Fort Worth is a member of the inter-jurisdictional Area Metropolitan Ambulance Authority (AMAA), the AED units are to be purchased only for use by the Fort Worth Fire Department. FINANCING• Sufficient funds are available for transfer in Capital Projects Reserve Fund 10, Project No. 010001-00, Unspecified. Upon approval and completion of Recommendation No. 1, sufficient funds will be available in General Fund O1, Account No. 36-80-00, Fire Training. Expenditures will be made from Index Code 279018. The balance in the Capital Projects Reserve Fund 10 after this transfer will be $2,230,952.00.
