AGREEMENT CITY SECRETARY CONTRACT NO. 46820 P WHEREAS, the City of Fort Worth ("City") and Deaf Link, Inc. ("Consultant"), collectively referred to as the "parties,"made and entered into City of Fort Worth City Secretary Contract ("CSC") No. 46820 on or about July 1, 2015, referred to herein as the "Agreement"; and WHEREAS, pursuant to the Agreement, the Consultant provides professional Consulting services for the purpose of providing Accessible Hazard Alert System (AHAS) and the AHAS Individual Notification(AHAS-IN)program in order to provide accessible emergency alerts to citizens with sensory disabilities; and WHEREAS, the Initial Term of the Agreement expires on July 1, 2016; however, to prevent a lapse in services,the subscription services began on April 6,2015;and WHEREAS, the City has the right to renew the Agreement for an additional term of one year under the same terms and conditions; and WHEREAS,the City has given proper notice of its intent to exercise such renewal. NOW THEREFORE, City and Consultant, acting herein by and through their duly authorized representatives, enter into the following agreement: 1. Section 1 of the Agreement shall be modified to include the services set forth in the attached Program Quote dated January 25,2016. 2. Section 2 of the Agreement shall be modified to amend the Term with the Initial Term beginning and being effective on April 6, 2015 ("Effective Date")and expiring one year from the Effective Date, and extend the Agreement with the Renewal Term commencing on April 6, 2016 CSC No.46820,Amendment and Renewal Deaf Link.Inc. OFFICIAL RECORD Pagel of 3 CITY SECRETARY FT.WORTH,TX and expiring on April 6, 2017 ("Renewal Term"). There shall be no further renewal options under the Agreement. 3. The compensation for the Renewal Term shall not exceed $44,944.00. 4. All other provisions of the Agreement that are not expressly amended herein shall remain in full force and effect. Page 3 of 3 at DEAF LIN K Accessible Hazard Alert System AHASTM Program quote for City of Fort Worth,TX Proposal dated: 1-25-2016 (revised) Deaf Link seeks to renew the contract with the City of Fort Worth for provision of the Accessible Hazard Alert System(AHASTM).The performance period is 12 months beginning April 6,2016 through April 5,2017. The program includes the following elements. 1. Deaf Link's AHASTM program will deliver accessible alerts and other emergency information at the direction of the Fort Worth Office of Emergencygement. 2. Deaf Link will send accessible web based alerts in American Sign Language(ASL)video,with English Voice,and Text(maximum suggested length 2 minutes- 150 words).ASL video alerts are created in Broadcast quality formats for possible use by local TV stations. 3. Deaf Link will provide 24/7 capability to send AHAS alerts in response to all Emergency Alert System(EAS)event codes for Warnings,Watches,and Emergencies,Civil Emergency Messages (CEM),Terrorism events,Amber Alerts,and National Weather Service(NWS)watches and warnings. 4. AHAS alerts include Short Message Service(SMS)capability for delivery of text based alerts to cell phones,web,or other mobile communication systems.Alert messages will have alert title with associated ASL video link embedded in the text. 5. The AHAS program is a subscriber based service allowing Deaf Link to deliver accessible alerts to computers,tablets,web/video capable phones,web capable braille readers and other devices as selected by the subscriber.This service is free to the subscriber other than any carrier costs associated with their broadband(phone or internet)service provider. 6. Deaf Link will add a Preparedness page to the FTW AHAS website.This feature will provide Fort Worth with a web location where accessible preparedness videos can be viewed by the community. 7. Community outreach program: Deaf Link can work with the City of Fort Worth in hosting community meetings to encourage personal preparedness and enrollment in the A14AS program.Meeting locations are determined and paid for by the City,broadband access for demonstrating AHAS is recommended. Meetings are a scheduled maximum of 2 hours in length.Deaf Link will charge$1500 per meeting which includes travel and provision of ASL interpreters. 8. The ShelterLinkTM program provides video based American Sign Language interpreting over the internet as well as Phone based interpreting(130 Spoken Languages)to support interpreting needs at Shelters. Costs for ShelterLink services are only incurred when FTW OEM requests activation of the program from Deaf Link. Costs: $100 Network/Set-up fee per client device,$500 p/day ASL interpreting,$2.30 p/minute phone based interpreting,all services available 24/7. Note: Deaf Link is sole source for the Accessible Hazard Alert System AHASTM program.
