LEASE AMENDMENT FOR A LEASE
BETWEEN THE CITY OF FORT WORTH AND
THE POTTER'S HOUSE OF DALLAS, INC.
WHEREAS, the City of Fort Worth, a Texas home rule municipal corporation ("Lessee")
entered into a lease agreement ("Lease") (City Secretary Contract No. 41587) effective as of
March 1, 2011, with The Potter's House of Dallas, Inc. ("Lessor") for leased premises located at
1200 Woodhaven Blvd. #1258, Fort Worth, Texas, more particularly described in the Lease
("Leased Premises): and
WHEREAS, Lessee and Lessor desire to amend the lease to extend the primary lease
term to September 30, 2016 raider the same terms and conditions and add (5) automatic
successive one year renewals contingent upon Crime Control and Prevention District funding.
NOW THEREFORE. in consideration of the rentals set forth in the Lease, the parties
agree to amend the lease as follows:
Lease Term. Section 3 of the Lease is hereby amended to change the date on which the
Term terminates from February-29, 2016 to September 30, 2016.
Renewals. Section 3 of the Lease is hereby amended to add the following language after
the existing paragraph:
"The Term shall automatically renew for up to five (5) successive one-year renewal
terms: provided, however, each renewal shall be contingent upon approval of Crime Control and
Prevention District ("CCPD") funds for the next one-year renewal term. If CCPD funding is not
approved and alternative funding is not identified by Lessee, Lessee will notify Lessor that the
lease will expire at the end of the then-current renewal term."
All other terms and conditions of the Lease remain unchanged and in full force and
effect.
This Lease Amendment is effective as of March 1, 2016.

SUBJECT: Authorize Execution of an Amendment to Extend the Lease Agreement with The Potter's
House of Dallas, Inc., for the Office Space Located at 1200 Woodhaven Boulevard, Suite
1258, for the Police Department Until September 30, 2016 in the Amount of$700.00 with
Five Successive One Year Renewals at a Cost of$1200.00 Per Year for a Total Cost of
$6,700.00 (COUNCIL DISTRICT 4)
RECOMMENDATION:
It is recommended that the City Council authorize the City Manager to amend the lease agreement
with The Potter's House of Dallas, Inc., for the approximately 1,625 square feet for office space
located at 1200 Woodhaven Boulevard, Suite 1258, for continued use by the Police Department as a
storefront location until September 30, 2016 in the amount of$700.00 with five automatic successive
one year renewals at a cost of$1,200.00 per year, for a total cost of$6,700.00.
DISCUSSION:
On March 1, 2011, the former Housing and Economic Development Department, at the request of the
Police Department, negotiated a lease for office space located at 1200 Woodhaven Boulevard,
Suite 1258 to be used as a Police Department storefront for a term of five years, beginning March 1,
2011 and expiring on February 29, 2016. There is a rental charge of$1,200.00 per year for the
space. (CSC No. 41587, M&C L-15147)
The Police Department with The Potter's House of Dallas, Inc.'s approval, has requested to extend
the current lease term to September 30, 2016 and will automatically renew for up to five successive
one-year renewal terms. Each renewal shall be contingent upon approval of Crime Control and
Prevention District (CCPD) funds for the next one-year renewal term.
If CCPD funding is not approved and alternative funding is not identified, the City of Fort Worth will
notify The Potter's House of Dallas, Inc., that the lease will expire at the end of the then-current
renewal term.
The prorated amount from March 1, 2016 to September 30, 2016 is $700.00. The total rent amount
for each fiscal year renewal is $1,200.00. The total amount for the extension and entire five one year
renewals is $6,700.00.
The City of Fort Worth is responsible for all utility and telephone payments.
This property is located in COUNCIL DISTRICT 4.
FISCAL INFORMATION/CERTIFICATION:
The Director of Finance certifies that funds are available in the current operating budget, as
appropriated, of the Crime Control and Prevention District Fund.
