C. Green Scaping,LP was unable to perform construction activities during two time suspension periods:(1)October 23,2015 to December 8,
2015 and(2)December 21,2015 to January 11,2016. These time suspensions were the result of the following:
1. Several design issues on original pians related to the street lighting design.
2.City had to resolve several issues with the street lights:what style or model number of pedestrian lights were going to be installed,supply of
anchor bolts and templates,relocation of foundations to avoid tree drainage,additional bore on northwest quadrant of project,and removal of
additional trees for light installations.
3.City had to work with TxDOT'to revise concrete mix design.
4.Tree and light locations need to be adjusted/omitted to accommodate current businesses drive approaches.
5.Design change on tree species.
In addition to the time suspensions above,C.Green Scaping,LP bid this project to work on both sides of the street simultaneously. During a
community meeting,the City instructed Green Scaping that they could only work on one half of the road before starting work on the other half.
Due to this direction,Green Scaping requests an additional 60 working days to complete the project.
The City of Ft.Worth and C.Green Scaping, LP are both in agreeance on the additional 60 working days requested.

The notice to proceed for the Hemphill/Berry Urban Village streetscape project was issued on August 4,
2015 at the pre-construction meeting for the project. During the meeting,Stacy Geigenmiller and Tracy
Lemoine of C.GreenScaping stated that construction would start on September 1, 2015. At a later date,
C.GreenScaping, Inc. informed the City that the new construction start date would be September 14,
2015.
This letter is to inform you that the City started time on the project effective Monday,September 14,
2015. In accordance with your contract,fully executed DBE and non-DBE subcontracting agreements
and/or work and purchase orders are required to be submitted to the City for TXDOTrs approval. Work
must not commence by DBE subcontractors until TxDOT has approved the submitted documentation.

As you are aware,time was suspended on Hemphill/Berry Village project on 10-28-2015. Please be
advised that the suspension was lifted effective 12/07/2015 and that Green Scaping is to resume work
on the project since all the issues listed below have been resolved:
1. City of Ft. Worth needed to decide on what style or model number of pedestrian lights were
going to be installed
2. City of Ft. Worth needed to supply Green Scaping with anchor bolts and templates to be
installed with the light piers
3. Design change needed for lights to be adjusted so piers would not penetrate tree drainage
4. Bore needed to be added to save established Red-Oak trees on the northwest of project
5. City of Ft.Worth needs to remove additional trees for light locations
6. Decision change was made for all light foundations to be 24" diameter, no 18" piers will be
installed
7. Tree and light locations need to be adjusted/omitted to accommodate current businesses drive
approaches
8. Clarification on removing microsurfacing line item

Dear Tracy:
As previously discussed, due to the City needing to change the concrete mix design for the
street light pole foundations to a TxDOT approved mix design, the Hemphill and Berry Urban
Village Streetscape Improvements project is being suspended until the new concrete mix design
has been approved by TxDOT. The suspension dates are from December 21, 2015 to January
11, 2016. Contract time will start on January 12, 2016.
Please provide via BuzzSaw the current project schedule and use the email feature to notify the
City's Hemphill Berry project members.
Prior to the start of construction, a street use permit for the traffic control is required. Please
contact Charles "Chuck" McLure, 817-392-7219, for the street use permit.
If you have questions about the contract time or matters related to construction, please contact

Contract Time Extension
The City is submitting this change order to request a time extension of 60 working days to the
current contract time of 65 working days. The following summary provides a detailed
explanation of the time line of the project and how the schedule has been impacted to justify this
request.
Dena Johnson, Project Manager, issued official Notice to Proceed for
79/15/2015 construction to begin,and contract time commenced. There are 65 working
days in the contract.
Time Suspension#1:
