PROGRAM AGREEMENT
HEALTH PROMOTION
The agreement is executed on 7/31/2000, between the University of North Texas for and on behalf of
the Health Promotion Program for their students,enrolled in the Baccalaureate and Master's degree
program of Health Promotion,and Fort Worth Public Health Department
Whereas, it is agreed by the parties that,
The purpose of the baccalaureate program in health promotion is to educate professional students to
promote healthy lifestyle changes within the community,and
The purpose of the master's program in health promotion is to promote health in areas of program
development,promotion and administration to contribute to healthy lifestyle changes within the community,
and
Whereas, it is agreed by the parties hereto to be of mutual interest and advantage that students and
faculty of Health Promotion be given the opportunity to utilize Fort Worth Public Health Department
henceforth referred to as "Facility") for education purposes:
The parties to this agreement agree as follows:
The students receiving internship experience in the Facility will be students in the undergraduate and
graduate health promotion program.
The students will be in the Facility during the regular and academic year defined as Fall,Spring and
Summer Semesters and will be under the academic supervision of a Health Promotion Faculty member.
Facility shall be responsible for direct on-site supervision and at all times retain responsibility for its
patients/clients.
The names of students and period of assignment will be furnished to the Facility by the Health
Promotion Internship Coordinator.
The number of students assigned to the Facility will be mutually agreed upon between the Health
Promotion Program and Facility.
The Facility further agrees to plan jointly with the Health Promotion Internship Coordinator as to
the type of experiences the student will receive.
Faculty and students will be subject to the rules and regulations established by the Facility to which
they are assigned.
Students will be responsible for their own expenses in the performance of this agreement.Students
will be volunteers at the Facility. There will be no exchange of monies between the Facility or the University
or any student and the Facility.
Students being assigned will be negotiated each academic semester. This program agreement is for
one year unless terminated by either party upon giving written notice to the other party.
The University shall be responsible for its actions,the students responsible for their actions,and the
Facility responsible for its actions.
The University and Facility shall not unlawfully discriminate in their respective performance of this
contract.
