C TO CITY SECRETARY CONTRACT
WHEREAS, the City of Fort Worth (CITY) and DeOtte,
Inc. (ENGINEER) made and entered into City Secretary Contract
No. 41902, (the CONTRACT) which was authorized by M&C C-24860
on the 12t�h day of April, 2011 in the amount of $84, 904 . 00 ; and
WHEREAS, the CONTRACT was subsequently revised by
Amendment No.l in the amount of $26,745 . 00 which was
administratively authorized on September 28, 2015; and
WHEREAS, the CONTRACT involves engineering services for
the following project:
Water Improvements on Ridglea Country Club Drive; and
WHEREAS, it has become necessary to execute Amendment
No.2 to the CONTRACT to include an increased scope of work and
revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and
through their duly authorized representatives, enter into the
following agreement, which amends the CONTRACT:

Article I of the CONTRACT is amended to include the
additional engineering services specified in a proposal dated
June 20, 2016, a copy of which is attached hereto and
incorporated herein. The cost to City for the additional design
services to be performed by Engineer totals $19, 946. 00 .
2 .
Article II of the CONTRACT is amended to provide for an
increase in the fee to be paid to Engineer for all work and
services performed under the Contract, as amended, so that the
total fee paid by the City for all work and services shall be
an amount of $131, 145 .00 .
3.
All other provisions of the Contract, which are not
expressly amended herein, shall remain in full force and
effect.

Dear Mr. Kasavich,
This letter is in reference to a contract between DeOtte, Inc. (DI) and the City of Fort
Worth (the City) dated June 22, 2011 and amended September 15, 2015 herein referred
to as the "Contract."
This letter requests an amendment revising the previously approved subject project
design plans and contract documents. Specifically, this amendment includes the
provision of additional work associated with the following items;
1. Additional site survey and topography due to extended project limits and added
project work items (cut-in valves)
2. Additional temporary and permanent construction/access easements for creek-
waterline crossing
3. Engineering labor associated with coordination of additional survey items
4. Engineering labor associated with coordination with City of Fort Worth
Stormwater Division
Additional Site Survey and Topography
The original contract work associated with waterline replacement was located along
Ridglea Country Club Drive between its intersection with Tamarack Road south to the
northern intersection of Coral Circle. During the 60% and 90% design phase of the
project, at the direction of the City, two additional cut-in water valves were proposed
along existing water lines in Tamarack Road and Ridglea Country Club Drive (near the
south intersection with Coral Circle). This revision necessitated additional site survey,
property research, and topography at each location as well as plan revisions to include
the added project sites. The original survey limits stopped approximately 80 feet west of
the north intersection of Ridglea CC Drive and Coral Circle. This amendment accounts
for the continuation of this survey approximately 800 feet farther west to include
Ridglea CC Drive and the south intersection with Coral Circle as well as approximately
150 feet along Tamarack Drive near its intersection with Streamwood Road.

Additional Temporary and Permanent Construction Easements
Construction access to the existing 16"water line crossing Mary's Creek is limited by
geography and topography.At the direction of the City,DI investigated site access
through site visits and property research to determine the required permanent and
temporary access easements required to enter the project site. There are a total of 6
required easements,exceeding the original project scope of 2 easements. The
preparation of the additional 4 easement documents shall be executed by a Sub-
Consultant.

2. Exhibits, Site Visits, &Coordination w/ Sub-Consultant $1,200.00
Coordination with Stormwater Division
Upon review of the proposed design during a field visit, the City directed that the
existing 16" water line crossing Mary's Creek be armored in place rather than replaced
by new construction. Since the design involves limited earthwork and placement of
erosion control structures within the creek,the creek crossing designs will require
evaluation by the City of Fort Worth Stormwater Division.

We recommend the City increase the budget by$14,496.00 for these additional
services. The following tables summarize the recommended budget revisions.
