AN ORDINANCE INCREASING ESTIMATED RECEIPTS AND
APPROPRIATIONS IN THE GRANTS FUND IN AN AMOUNT UP TO $100,000,
SUBJECT TO RECEIPT OF THE GRANT FROM THE TEXAS DEPARTMENT OF
STATE HEALTH SERVICES, FOR THE PURPOSE OF FUNDING THE PUBLIC
HEALTH DEPARTMENT IMMUNIZATION OUTREACH TO THE COMMUNITY
FROM SEPTEMBER 1, 2008 TO AUGUST 31, 2009; PROVIDING FOR A
SERVERABILITY CLAUSE; MAKING THIS ORDINANCE CUMULATIVE OF
PRIOR ORDINANCES IN CONFLICT HEREWITH; AND PROVIDING AN
EFFECTIVE DATE.
BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF FORT WORTH,
TEXAS:
SECTION 1.
That in addition to those amounts allocated to the various City departments for Fiscal Year 2007-
2008 and in the Budget of the City Manager, there shall also be increased estimated receipts and
appropriations in the Grants Fund in an amount up to $100,000, subject to receipt of the grant from
the Texas Department of State Health Services, for the purpose of funding the Public Health's
Department Immunization Outreach to the Community from September 1, 2008 to August 31, 2009.
SECTION 2.
That should any portion, section or part of a section of this ordinance be declared invalid,
inoperative or void for any reason by a court of competent jurisdiction, such decision, opinion or
judgment shall in no way impair the remaining portions, sections, or parts of sections of this
ordinance, which said remaining provisions shall be and remain in full force and effect.
SECTION 3.
That this ordinance shall be cumulative of Ordinance No. 17749 and all other ordinances and
appropriations amending the same except in those instances where the provisions of this ordinance
are in direct conflict with such other ordinances and appropriations, in which instance said
conflicting provisions of said prior ordinances and appropriations are hereby expressly repealed.
SECTION 4
This ordinance shall take effect upon adoption.

Authorize Application for and Acceptance of Texas Department of State Health Services Grant to
Provide Immunization Outreach to the Community from September 1, 2008, to August 31, 2009,
Authorize Execution of Grant Contract, Waive Indirect Costs, Authorize an Interfund Loan and Adopt
Appropriation Ordinance
RECOMMENDATION:
It is recommended that the City Council:
1. Authorize the City Manager to apply for and accept , if awarded, a grant from the Texas Department of
State Health Services for up to $100,000.00 to be used to increase immunization levels in birth to 18 year
old children and provide awareness and education at the community level, from September 1, 2008, to
August 31, 2009;
2. Authorize execution of grant contract with Texas Department of State Health Services for period
September 1, 2008 through August 31, 2009;
3. Waive indirect costs of 17.59 percent of related salary and fringe benefits which are the most recent rates
indicated by the Finance Department;
4. Authorize anon-interest bearing interfund loan from the General Fund to the Grants Fund in the amount
of $25,000.00 for the interim financing of this grant pending contract execution to provide for a smooth
transition fram one grant funding to the next; and
5. Adopt the attached appropriation ordinance increasing estimated receipts and appropriations by an
amount up to $100,000.00 in the Grants Fund.
DISCUSSION:
The 2006 National Immunization Survey, which tracks immunization rates among preschool children, found
that Texas coverage levels declined from 76.8 percent in 2005 to 74.7 percent in 2006. The Healthy People
2010 goal for individual vaccines is 90 percent.
The National Immunization Survey provides vaccination coverage estimates for children 19 through 35
months of age. State rankings are based on the percentage of children completing the 4:3:1:3:3:1 series of
immunizations. That series includes four doses of Diphtheria, Tetanus, and Pertussis (DTaP), three doses
of Polio Vaccine, one dose of Measles-containing vaccine, three doses of Hib vaccine, three doses of
Hepatitis B vaccine, and one dose of Varicella vaccine.
The fundamental reasons for under immunization are factors related to poverty. These factors can be
overcome by removing financial and access barriers. The City's Public Health Department, Outreach

Division, is proposing a continued partnership with the Fire Department to immunize children birth to 18
years of age. This partnership with emergency personnel makes sense for our community. The personnel
are medically trained, know their neighborhoods, and the fire stations are known "safe havens" in the
community. The fire stations are easily accessible, Americans with Disabilities Act compliant, and
appealing to young children. Flexible hours of operation help remove the barrier of access for working
parents. Enlisting firefighters and paramedics to become immunization partners is an innovative way to
keep improving immunization coverage in this vulnerable population. The end result is protection for all the
residents of Fort Worth. There will continue to be no charge to the citizens for these immunizations.
The goal of this proposal is to utilize immunization funds to achieve and maintain 90 percent vaccine
coverage for vaccine-preventable diseases. Currently vaccines are given for DTaP (tetanus, diphtheria,
and acellular pertussis), IPV (inactivated poliovirus vaccine), MMR (measles, mumps, and rubella), HepB
(Hepatitis B), HepA (Hepatitis A), Varicella (chickenpox), Hib (haemophilus influenza type b), PVC 7
(Pneumococcal Disease), Rotavirus, MCV-4 (Meningococcal), HPV (human papillomavirus vaccine), Td
(Tetanus/Diphtheria), Tdap (Tetanus/Pertussis). The immunization program has provided 11,059 vaccines
to 3,748 children from January 2002, through February 2008. Of these children, 2481 (66.2 percent) were
not insured, 157 (4.2 percent) were underinsured, 982 (26.2 percent) were on Medicaid, 109 (2.9 percent)
were on the Children's Health Insurance Program (CHIP), and 19 (.5 percent) had unknown insurance
coverage.
As part of the advocacy role, Outreach staff is aware of the lack of culturally competent access to
services. Immunization services must be provided in a way that encourages compliance by meeting the
needs of the population at highest risk for not being immunized. The continuation of this immunization
program will help ensure access.
The current locations for this program are Station 15 (3100 Azle Avenue), Station 17 (212 East Felix
Street), and Station 3 (4700 Ramey). Protocol and standing delegation orders are in place and quality
assurance is maintained through a collaborative effort between the Public Health Department and the Fire
Department.
The Hot Shot Immunization Program has the ultimate goal of providing immunizations for all demographic
areas of Ft. Worth. At the present moment the Hot Shot Immunization Program covers the North, South,
and East side of town. We are requesting additional funding from the Department of State Health Services
(DSHS) to open a fourth clinic location at an existing fire station on the west side of town. The majority of
our clients utilize public transportation and have to travel across town to receive their
immunizations. Having a fire station in their neighborhood will decrease the barriers that we face as a state
with our low immunization rates. An addition of the fourth fire station will ensure that we are providing care
to the North, South, East and West side of Ft. Worth. The majority of the cost will come from the overtime
rate of the FF/Paramedic.
The Outreach staff distributes information about the availability of services throughout the City in recreation
centers, day care centers, child care provider classes, and faith based organizations, for example. Data
collection is done using the Statistical Package for the Social Sciences (SPSS) Version 14.0. Data is
entered into the Tarrant County Immunization Registry and downloaded to the State's "ImmTrac" data base.
Staff recommends the waiver of indirect costs to maximize the funds toward the grant objective.
FISCAL INFORMATION/CERTIFICATION:
The Finance Director certifies that upon approval of the above recommendations and adoption of the
attached appropriation ordinance, funds will be available in the current operating budget, as appropriated, of
the Grants Fund. Upon execution of the grant contract, the interim inter-fund loan will be returned to the
General Fund.
