HomeMy WebLinkAboutOrdinance 8652y b r -- :: ORDINANCE NO. ~~ ,~a AN ORDINANCE AMENDING CHAPTER 19, "HEALTH AND SANITATION," OF THE CODE OF THE CITY OF FORT WORTH (1964), AS AMENDED, PROVIDING FEES FOR SERVICES RENDERED BY THE CITY REGISTRAR OF VITAL STATISTICS; PROVIDING FEES FOR PUBLIC HEALTH SERVICES RENDERED BY THE PUBLIC HEALTH LABORATORY, THE SEXUALLY TRANSMITTED DISEASE CLINC, THE IMMUNIZATION CLINIC, AND THE DENTAL CLINIC, AND GRANTING THE DIRECTOR OF THE DEPARTMENT OF PUBLIC HEALTH AUTHORITY TO ESTABLISH GUIDELINES WAIVING SAID FEES FOR THE INDIGENT; AUTHORIZ - ING PUBLICATION IN PAMPHLET FORM; MAKING THIS ORDINANCE CUMULATIVE OF PRIOR ORDINANCES; REPEALING ALL ORDINANCES AND PROVISIONS OF THE FORT WORTH CITY CODE IN CONFLICT HEREWITH; PROVIDING A SEVERABILITY CLAUSE; AND DECLARING AN EFFECTIVE DATE BE IT ORDAINED BY THE CITY COUNCIL OF THE CITY OF FORT WORTH, TEXAS 4 SECTION 1 That there be established a Section 19-21 1 of Chapter 19 of the Code of the City of Fort Worth (1964), as amended, the same to read and b e as follows "Sec. 9-21. 1 Vital Statistics Fees. The City Registrar of Vital Statistics is en-• titled to a fee of five dollars ($5 00) for each copy of a birth record or part of a record and five dollars ( $5 00) for the first copy of a death record and two dollars ($2 00) for each additional copy of the record requested by the applicant in a single request. For any search of the files where a record is not found or a certified copy is not made, the city registrar shall be entitled to a fee of five dollars ( $5. 00) , said fee to be paid by the applicant SECTION 2 That there be established a Section 19-440 1 of Chapter 19 of the Code of the City of Fort Worth (1964), as amended, the same to read and b e as follows "Sec. 19-440.1. Public Health Service Fees. The Department of Public Health shall be entitled to collect the following fees from those individuals using the facilities and/or services of the department as listed below (a) Public Health Laborator - (1 bacteriological analysis of standard water sample $ 5 00 (2) chemical analysis of standard water sample $50 00 ~~. SECTION 6 It is hereby declared to be the intention of the City Council that the sections, paragraphs, sentences, clauses and phrases of this ordinance are severable, and if any phrase, clause, sentence, paragraph or section of this ordinance shall be declared unconstitu- tional by the valid judgment or decree of any court of competent jurisdiction, such unconstitutionality shall not affect any of the remaining phrases, clauses, sentences, paragraphs and sections of this ordinance, since the same would have been enacted by the City Council without the incorporation in this ordinance of any such unconstitutional phrase, clause, sentence, paragraph or section.
