FOR DESIGN OF THE CAHOBA DRIVE WATER LINE STATE OF TEXAS § COUNTY OF TARRANT § WHEREAS, on February 23, 1999,the City of Fort Worth, Texas(hereinafter called"CITY"and Alan Plummer Associates, Inc. (hereinafter called "ENGINEER"), made and entered into City Secretary Contract No. 24760, which was duly authorized by the City Council by M&C C-17293 on February 23, 1999, and amended by M&C C-17794 on. December 21, 1999, City Secrotary Contract No. 25677, and, WHEREAS, the CITY and ENGINEER desire to amend City Secretary Contract No. 24760. NOW, THEREFORE, KNOW ALL MEN BY THESE PRESENTS: That the City of Fort Worth, a municipal corporation situated in Tarrant County,Texas, acting herein by and through its duly authorized City Manager, hereinafter called "CITY," and Alan Plummer Associates, Inc., acting herein by and through its duly authorized President, hereinafter called "ENGINEER," hereby make and enter into the following agreement: 1. EXHIBIT "A-6," SUPPLEMENTAL SCOPE OF SERVICES is hereby amended by adding the following: PART E — ADDITIONAL DESIGN SERVICES FOR THE CAHOBA DRIVE WATER LINE AND WOODVALE WATER EXTENSION 1. Prepare one plan and profile sheet for a detailed design of an 840-foot long, 16-inch water line extension of the Cahoba Drive Water Line. The proposed line begins at a tee on the Cahoba Drive Water Line at the intersection of Navarro Drive and Cahoba Drive and extends along the IH 820 frontage road. 2. Prepare, at the City's request, a design change for the extension of the Woodvale Water Extension to extend the 10-inch water line to Vinca Circle to serve seven houses with new pipeline. The seven houses are currently served by the Woodvale Water System 3. Provide 1,340 linear feet of additional construction staking for the extension of pipeline to Vince Circle. 4. Additional construction staking procedures along Woodvale Water Extension including the following: a. Set offset and grade for cuts on 2,000 linear feet, b. Revising and reprinting contract documents for agreement with M.E. Burns to complete the construction project. 2. EXHIBIT B - COMPENSATION AND PAYMENT SCHEDULE is hereby amended by revising Paragraph 1.A as follows: A. The Engineer shall be compensated a total lump sum fee of $202,091.41 as summarized in Exhibit"B-3." Payment of the total lump sum fee shall be considered full compensation for the services described in Attachment"A"and Exhibit"A-6"for all labor materials, supplies and equipment necessary to complete the project. The revised EXHIBIT "B-3" – DESIGN FEE COST SUMMARY is attached. 3. All other terms and conditions of City Secretary Contract No. 24760 not amended herein are unaffected and on time in full force and effect. Water Department Project No.PW77-060770153220 Cahoba Drive Water Line and Woodvale Water Extension Summary of Requests for Additional Fees for Extra Work Dear Ms. Rafalko: This letter serves as a summary of the work performed or to be performed for the additional fees we have requested for the Cahoba Drive Water Line and the Woodvale Water Extension projects. In a letter dated September 21,2000,we requested the following additional fees: Cahoba Drive Water Line: Design of an 840-foot long, 16-inch water line extension of the Cahoba Drive Water Line. The amount of fee requested for this item was$3,467.50. Woodvale Water Extension: Design of an addendum item detailing an additional 1340 feet of pipe. The amount of fee requested for this item was$4,716.00. This amount included construction staking fees of $1,926.00 for the additional length. Since September,we have performed additional work as follows: Additional Construction Staking for Woodvale Water Extension: The fees for construction staking for Woodvale Water Extension were based on the surveyor staking the centerline for the pipeline by locating the points of inflection. At the request of the City inspectors,Garcia and Associates provided additional staking services. Revise Contract Documents for M.E.Bums: The fees for revising the contract documents for M.E.Burns Construction,Inc.to complete the project are detailed as follows: Design Engineer-6.5625 Hours @$80.00= $ 525.00 Contract Document and Specification Reproduction(7 sets)_ $ 387.08 Plan Set Reproduction(7 sets)_ $ 196.58 The reproduction costs include a 15 percent markup. The total fees for revising the contract documents is$1108.66 The total amount of additional fees requested for all the above items is$13,478.41. We appreciate your consideration of this matter.
