GRANT STATEMENT
THIS GRANT STATEMENT is entered into and forms a part of that certain Grant
Agreement dated December 12, 2011 (the "Agreement") by and between PetSmart Charities, Inc.
("PetSmart Charities") and the "Organization" identified below. The Parties are entering into
this Grant Statement to set forth additional terms and conditions for a Grant from PetSmart
Charities to the Organization. Capitalized terms not otherwise defined herein shall have the
meanings set forth in the Agreement.

Grant Purpose: The Grant Funds will total $ 6,098.40 for the City of Fort Worth #1811 to
host an Adoption Weekend Event to take place on 7/16, 17 &18/2016 at
PetSmart Stores Stores 187, 2065 and 1122 with a goal of 125 adoptions
and participation by other approved PetSmart Charities Adoption Partners
may be agreed by written permission of PetSmart Charities. Grant funds will
be used for:
1. Approximately $1,567 in Marketing/Advertising for the Adoption
Weekend Event. Marketing and advertising include: social media, television
and radio spots, billboards, newspaper ads, banners, and flyers.
Organization will use provided event-specific PetSmart Charities
promotional materials. Any marketing materials created by the Organization
shall be submitted to PetSmart Charities for review, prior to use, including
any and all promotional or advertising materials, press releases, written
communication and/or social media posts in which PetSmart Charities'
name, trademarked material or copyrighted material is used, at least 10
business days in advance of advertising and print deadlines.
2. Approximately $4,531.40 for permits, insurance, security, tables,
chairs, tents, fencing and/or other similar items as described within your
Grant Application. If tent(s) are described with your grant request, included
will be needed supplies to ensure adequate preparation for potential
inclement weather, up to and including: puppy pools, tent panels, heaters,
air conditioning, fans, etc.
3. If any grant funds remain after the expenses in items (1) through (2)
above are covered, the remaining grant funds may be used, with written
permission from PetSmart Charities, Inc. toward spay/neuter surgeries for
animals to assist in preparing them for adoption.
Grant Period: The Grant Period will commence on the Effective Date set forth in
this Grant Statement, which in no event shall be earlier than effective
date of the Agreement and will continue for a period of six months
after full execution. .
Reporting Obligations: As required by Adoption Partner Standard Operating Procedures as
documented in the PetSmart Store Adoption Partner Manual
In addition to any reporting requirement set forth in the Master Support
Agreement and/or the In-Store Adoption Partner, the Organization agrees to
provide the following reports to PetSmart Charities in connection with this
event grant, on or before the deadlines set below. PetSmart Charities
reserves the right to change the method and format of how reports are
provided.
Unless otherwise specified, submit all reports via
www.cybergrants.com/petsmartcharities/reports/app. The final Adoption
Event Impact Report and the Expenditure Re ort templates are available at

By 10/18/2016 or no later than 30 days after the completion of the event,
whichever comes sooner, a Final Adoption Event Impact Report including
1) A detailed report on the expenditures of Grant Funds. A detailed
report on the total Income and expenses for the project or event funded by
the grant.
2) A final project or event report on the on the results and impact of
the project or event, including
(A) Total number of pets adopted broken down by:
(i) Total number of adult cats available for adoption at the event
(ii) Total number of adult cats adopted at the event
(iii) Total number of kittens 5m and under available for adoption at the
event
(iv) Total number of kittens 5m and under adopted at the event
(v) Total number of adult dogs available for adoption at the event
(vi) Total number of adult dogs adopted at the event
(vii) Total number of puppies 5m and under available for adoption at the
event
(viii) Total number of puppies 5m and under adopted at the event
(ix) Total number of other animals (rabbits, ferrets, etc.) adopted at the
event

Additional Terms: As of the date of signing this Grant Statement, the Organization
represents to PetSmart Charities that each of its representations,
warrants and covenants provided in the Agreement and this Grant
Statement are true and correct, including, but not limited to, those set
forth in Section 8 of the Agreement.
This Grant Statement shall not be binding on either Party until executed by both Parties.
Organization acknowledges and agrees that PetSmart Charities may rescind any offer contained
herein at any time until the Agreement has been executed by both Parties.

HEARING:
SUBJECT: Authorize Application for and Acceptance of Multiple Grants in an Amount Not to Exceed
$150,000.00 from Animal Welfare Organizations Including PetSmart Charities, Inc., to
Support Animal Welfare Programs Including Pet Adoption Events Held Throughout the
Year, Waive Indirect Costs and Adopt Appropriation Ordinance (ALL COUNCIL
DISTRICTS)
RECOMMENDATION:
It is recommended that the City Council:
1. Authorize the application for and the acceptance of grants in the amount up to $150,000.00 from
animal welfare organizations including PetSmart Charities, Inc., to pay the expenses generated from
multiple pet adoption events and other animal welfare programs;
2. Waive the imposition of any indirect costs as part of the City of Fort Worth's in-kind match to these
grants; and
3. Adopt the attached appropriation ordinance increasing estimated receipts and appropriations in
the Grants Operating Other Fund in the amount of$150,000.00 upon receipt of grants.
DISCUSSION:
The City of Fort Worth enjoys a productive relationship with PetSmart Charities, Inc., that includes
working together to promote animal welfare through numerous special initiatives for the benefit of Fort
Worth animal shelter animals. In addition to the ongoing partnership to support two satellite adoption
centers at PetSmart retail stores, PetSmart Charities, Inc., and the City's Code Compliance
Department periodically coordinate special adoption or other promotional events in Fort
Worth. These events are typically facilitated by the City's Code Compliance Department with an
Agreement that PetSmart Charities, Inc., will reimburse the City for costs associated with the event,
which can include marketing, promotion and purchase/or rental of event equipment and supplies.
These Agreements have historically been authorized by City Council on an event-by-event basis,
resulting in multiple Mayor and Council Communications (M&C) each year for the same kinds of
events. This year, in the interest of efficiency, the department is requesting approval for the year's
adoption events from animal welfare organizations and reimbursements including those by PetSmart
Charities, Inc., as a single M&C.
The Code Compliance Department will apply for and accept animal welfare organization grants
including those from PetSmart Charities, Inc., and reimbursements for special events in the amount
up to a total of$150,000.00 to facilitate a more efficient and timely turnaround of application and
grant or Agreement documents.
This M&C does not request approval of a contract with a business entity.

FISCAL INFORMATION/CERTIFICATION:
The Director of Finance certifies that upon approval of the above recommendations, receipt of the
grant(s) and adoption of the attached appropriation ordinance, funds will be available in the current
operating budget, as appropriated, of the Grants Operating Other Fund. Funds will be made
available by the Grantor after both Grantor and the City have signed the Grant Agreement.
