(No M&C Needed) COUN'T'Y OF TARRANT S WHEREAS, the City of Fort Worth (City) and Carter & Burgess, Inc. , (Engineer) made and entered into City Secretary Contract No. 28236, (the Contract) which was authorized by the City Council by M&C C-19346 on the 12th day of November 2002 and subsequently revised by two previous amendments; and WHEREAS, the Contract involves engineering services for the following project: Engineering contract for the design of Sanitary Sewer Rehabilitation Contract LXVIII (68) . WHEREAS, it has become necessary to execute Amendment No. 3 to said Contract to include an increased scope of work and revised maximum fee; NOW THEREFORE, City and Engineer, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1. Article I of the Contract is amended to include the additional engineering services specified in a proposal letter dated April 21, 2004, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer total $4, 587 .00 . 2 . Article II of the Contract is amended to provide for an increase in the maximum fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall not exceed the sum of $239,205 .75. No. 3766 Carter& Burgess No. 0 11625.010 Dear Mr. Domenech: At the request of the City of Fort Worth, we are relocating M-154 to the street. This effort requires additional survey that was provided by A.N.A. Consultants. This redesign and relocation effort is not part of the original contract agreement. Therefore, we request that you approve this contract amendment. The following is a breakdown of the compensation for this task: Engineering Design Fee $3,510.00 Additional Survey Fee $ 600.00 10% Contingency Fee $ 417.00 10% MWBE Administration Fee $ 60.00 Total Amendment 3 Request $4,587.00 This fee will cover our expenses for relocating approximately 307 L.F. of pipe to the street and relaying approximately 125 L.F. of pipe in the alley to redirect flow to the new alignment. This part of the contract was originally anticipated and intended to be a pipe burst job. The net change to our design is to add 432 If of open trench pipe construction and remove 125 If of pipe bursting. Because of this, survey was limited to locating cleanouts and a topographic survey of existing alignment. We would appreciate that this amendment request be approved in a timely manner.
