AMENDMENT No.4 CONTRACT N0. q
TO CITY SECRETARY CONTRACT
WHEREAS, the City of Fort Worth (CITY) and Bridgefarmer &
Associates, Inc. , (ENGINEER) made and entered into City Secretary
Contract No . 44165, (the CONTRACT) which was administratively authorized
by the City on the 1st day of February, 2013 in the amount of $39 , 300 . 00;
and
WHEREAS, the CONTRACT was subsequently revised by Amendment Number 1
in the amount of $393 , 767 . 65 authorized by the City Council by M&C C-
26422 on September 10, 2013 ; subsequently revised by Amendment Number 2
in the amount of $131, 637 .20 authorized by the City Council by M&C C-
26846 on July 22, 2014; and subsequently revised by Amendment Number 3 in
the amount of $39, 740 . 00 administratively authorized by the City on
August 8, 2014;
WHEREAS, the CONTRACT involves engineering services for the
following project :
Parker-Henderson Road from Mansfield Highway to David Strickland Road
WHEREAS, it has become necessary to execute Amendment No. 4 to the
CONTRACT to include an increased scope of work and revised fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:

Article I of the CONTRACT is amended to include the additional
engineering services specified in a proposal dated July 6, 2016, a copy
of which is attached hereto and incorporated herein. The cost to City
for the additional design services to be performed by Engineer totals
$478, 620 . 00 .
2 .
Article II of the CONTRACT is amended to provide for an increase in
the fee to be paid to Engineer for all work and services performed under
the Contract, as amended, so that the total fee paid by the City for all
work and services shall be an amount of $1, 803 , 064 . 85 .

Scope for Engineering Design Related Services for Street Improvements
The scope set forth herein defines the work to be performed by the ENGINEER in
completing the project. Both the CITY and ENGINEER have attempted to clearly define the
work to be performed and address the needs of the Project.
OBJECTIVE
The objective of this contract is to progress and complete design and construction plans for
Parker-Henderson Road. The project scope has been changed to include the following:
• Pulverize and overly the existing road from Mansfield Highway to Lifestyle Circle.
• Reconstruct the existing road from Lifestyle Circle to David Strickland with sidewalks
on both sides.
• Construct sidewalk on both sides of the road from David Strickland to the existing
bridge culvert.
• Replace the existing bridge culvert for flood improvements and provide sidewalks on
both sides of the road to Willow Springs Road.
• Construct sidewalks on the east side of the road from Willow Springs Road to East
Kellis Street.
This contract amendment includes work for the design of roadway, drainage, water,
structural, geotechnical, easements, survey, and permitting services required to change the
project limits and update all affected sheets. Additional bid phase services are also
included.

TASK 1. DESIGN MANAGEMENT.
ENGINEER will manage the work outlined in this scope to ensure efficient and effective use
of ENGINEER's and CITY's time and resources. ENGINEER will manage change,
communicate effectively, coordinate internally and externally as needed, and proactively
address issues with the CITY's Project Manager and others as necessary to make progress
on the work.
1.1. Manage and direct design team activities
• Ensure quality control is practiced in performance of the work
• Communicate internally among team members
• Task and allocate team resources

1.2. Communications and Reporting
• Conduct and document monthly project update meetings with CITY Project Manager.
• Conduct and document monthly design team meetings.
• Prepare invoices, in accordance with Attachment B to this Standard Agreement and
submit monthly in the format requested by the CITY.
• Prepare and submit monthly progress reports in the format provided by the respective
CITY Department.
• Prepare and submit baseline Project Schedule initially, and Project Schedule updates
as needed and according to the City of Fort Worth's Schedule Guidance Document.
• Complete Monthly M/WBE Report Form and Final Summary Payment Report Form at
the end of the project.
• Coordinate with other agencies and entities as necessary for the design of the
proposed infrastructure, and provide and obtain information needed to prepare the
design.
• With respect to coordination with permitting authorities, ENGINEER shall
communicate with permitting authorities such that their regulatory requirements are
appropriately reflected in the designs. ENGINEER shall work with regulatory
authorities to obtain approval of the designs, and make changes necessary to meet
their requirements, as part of the design scope.
• Personnel and Vehicle Identification: When conducting site visits to the project
location, the ENGINEER or any of its sub-consultants shall carry readily visible
information identifying the name of the company and the company representative.

TASK 4. FINAL DESIGN (90 PERCENT) AND FINAL CONSTRUCTION DOCUMENTS
(100 PERCENT).
4.1 Cast-in-place walls. Complete structural design for cast-in —place cantilever walls.
4.2 Jointing plan. Prepare jointing plan for construction of concrete pavement along the
Parker Henderson project limits.
4.3 Prepare replanting plan. Trees to be removed in front of the gas well site (Parcel
10PT2) will need to be mitigated (inch per caliper inch removed).

4.4 Prepare table of all trees to be removed including size, species and ID#.
4.5 Traffic Control Plans. Design effort to develop sequence of construction and traffic
control plans based on new scope of project.
4.6 Complete final design calculations for new bridge or culvert over Prairie Dog Creek
including hydraulic calculations, scour analysis, and grading.
4.7 Preparation of details for block wall and channel protection.
4.8 Bridge design at Prairie Dog Creek.

4.9 Updated water main improvement plans. Proposed water main will end at the limit of
asphalt pavement transition to the north of David Strickland.
4.10 Geotechnical Investigations. The ENGINEER shall perform soil investigations,
including field and laboratory tests, borings, related engineering analysis and
recommendations for determining soil conditions will be made for the design of a
bridge crossing at Prairie Dog Creek. Required information includes d50 and d90
grain size determination for scour analysis; L-Pile parameters to be used for bridge
foundation recommendations; and soil parameters to be used for the design of
channel protection.
The ENGINEER shall provide retaining wall design and construction
recommendations including global stability analysis.
The ENGINEER shall prepare a detailed engineering report summarizing the results
of the field and laboratory data. The report shall be prepared by a registered
engineer.
4.11 Drainage plans.
a. Update drainage area maps
b. Update drainage computation sheets
c. Update drainage plan and profile sheets
d. Design drainage ditches adjacent to proposed sidewalk. 100 year design to be
contained within ROW.
e. Design driveway culverts as needed.
4.12 Update plan sheets for revised project scope including:

4.13 Show relocated utilities on plan sheets.
4.14 Construction Cost. Update final quantities and unite prices for construction cost
estimate.
4.15 Specifications. Complete project final specifications documents.
TASK 5. BID PHASE SERVICES.
ENGINEER will support the bid phase of the project as follows.
5.1. Bid Support
• The ENGINEER shall upload all plans and contract documents onto Buzzsaw for
access to potential bidders.
• Contract documents shall be uploaded in a .xis file.
• Unit Price Proposal documents are to be created utilizing CFW Bidtools only
and combined in a specified spreadsheet workbook, Bid Proposal Worksheet
Template, and will be populated and configured so that all pages are
complete and the Summary Worksheet(s) in the workbook detail and
automatically summarize the totals from the inserted Unit Price Proposal
document worksheets.
• Plan Sets are to be uploaded to Buzzsaw in two formats, .pdf and Awf files.
The .pdf will consist of one file of the entire plan set. The Awf will consist of
individual files, one for each plan sheet, and will be numbered and named in
a manner similar to that of the plan set index.
• The ENGINEER shall sell contract documents and maintain a plan holders list on
Buzzsaw from documents sold and from Contractor's uploaded Plan Holder
Registrations in Buzzsaw.
• The ENGINEER will develop and implement procedures for receiving and
answering bidders' questions and requests for additional information. The
procedures shall include a log of all significant bidders questions and requests
and the response thereto. The log shall be housed and maintained in the
project's Buzzsaw folder titled Request for Information. The ENGINEER will
provide technical interpretation of the contract bid documents and will prepare
proposed responses to all bidders questions and requests, in the form of
addenda. The ENGINEER shall upload all approved addenda onto Buzzsaw and
mail addenda to all plan holders.

• Attend the prebid conference in support of the CITY.
• Assist the CITY in determining the qualifications and acceptability of prospective
contractors, subcontractors, and suppliers.
• When substitution prior to award of contracts is allowed in the contract
documents, the ENGINEER will advise the CITY as to the acceptability of
alternate materials and equipment proposed by bidders.
• Attend the bid opening in support of the CITY.
• Tabulate and review all bids received for the construction project, assist the CITY
in evaluating bids, and recommend award of the contract. A copy of the Bid
Tabulation and the CFW Data Spreadsheet created utilizing CFW Bidtools only
are to be uploaded into the project's Bid Results folder on Buzzsaw.
• Incorporate all addenda into the contract documents and issue conformed sets.
TASK 7. ROW/EASEMENT SERVICES.
ENGINEER will support and perform activities related to ROW and land as outlined below,
per scoping direction and guidance from the CITY's Project Manager
7.2. Right-of-Way/Easement Preparation and Submittal.
• The ENGINEER shall prepare documents to be used to obtain right-of-way and
permanent and/or temporary easements required to construct the improvements.
• The documentation shall be provided in conformance with the checklists and
templates available on the CITY's Buzzsaw site.
ASSUMPTIONS
• Additional easements may be needed for the proposed work as well as revisions
to previously prepared documents.
DELIVERABLES
A. Easement exhibits and meets and bounds provided on CITY forms.
TASK 8. SURVEY AND SUBSURFACE UTILITY ENGINEERING SERVICES.
ENGINEER will provide survey support as follows.
8.1. Survey Verification
ENGINEER will perform field surveys to collect horizontal and vertical elevations of
relocated utilities constructed in place.
Information to be provided includes X, Y and Z coordinates in the datum established for the
project, in a Microstation file format that can be referenced into the project Cadd files for the
design plan sheets.

TASK 9. PERMITTING.
ENGINEER will provide permitting support for the CITY to obtain any and all agreements
and/or permits normally required for a project of this size and type, as follows.
9.2. Environmental Services: US Army Corp of Engineers Section 404 Nationwide Permit
• Negotiating and coordinating to obtain approval of the agency issuing the
agreement and/or permits.
• Prepare wetland mitigation plan, if needed.
• Completing all forms/applications necessary.
• Submitting forms/applications for CITY review
• Submitting revised forms for agency review
• Responding to agency comments and requests
9.3. Floodplain Services:
• Meet, negotiate and coordinate to obtain approval of the agency issuing the
agreement and/or permits.
• Completing all forms/applications necessary.
• Submitting forms/applications for CITY review

9.4. TxDOT Permit:
• Revise and resubmit TxDOT permit according to revised project scope.

RECOMMENDATION:
It is recommended that the City Council authorize the execution of Amendment No. 4 in an amount not to
exceed $478,620.00 to City Secretary Contract No. 44165, an Engineering Services Agreement with
Bridgefarmer&Associates, Inc., for the design of Parker-Henderson Road from Mansfield Highway to
Martin Street for a revised contract amount not to exceed $1,083,064.85 (CPN C01419).
DISCUSSION:
The 2008 Capital Improvement Program allocated funds for the reconstruction of Parker-Henderson Road
from Mansfield Highway to David Strickland Road with the construction of sidewalks from David Strickland
Road to Martin Street. An Engineering Services Agreement, City Secretary No. 44165 with Bridgefarmer&
Associates, Inc. was authorized for the design of the project that included full concrete reconstruction of the
roadway from Mansfield Highway to Prairie Dog Creek, a new Bridge at Prairie Dog Creek and sidewalks
from the new bridge to Martin Street. Project costs to complete this project are estimated to substantially
exceed the project funding allocation of$4.7 million. Staff has reevaluated the scope of the project and
determined that modifications can occur that incorporate project elements that parallel the needs of the
community and maintain the project budget within the funding allocation of$4.7 million.
The proposed design Amendment is to revise the project scope to include the following improvements:
Pulverization and overlay of the existing pavement from Mansfield Highway to Lifestyle Circle. This will
reduce the quantity of new concrete pavement, but additional design effort is required to install drainage
ditches, driveway culverts, or other storm drain improvements within the existing right-of-way;
Full reconstruction of the Parker-Henderson Road from Lifestyle Circle to David Strickland Road including
sidewalks to link the abutting residential land uses to the elementary school and Prairie Dog Park;
Final design of box culverts to replace the existing bridge on Parker-Henderson Road over Prairie Dog
Branch;
Construction of sidewalks on the west side of Parker-Henderson Road from David Strickland Road to
Kellis Street. The previous project scope called for full reconstruction for this segment of Parker-
Henderson Road, which is being eliminated. Additional design effort is required to install drainage
ditches, driveway culverts, and storm drains within the existing right-of-way; and
Construction of sidewalks on the east side of Parker-Henderson Road from David Strickland Road to
Willow Springs Road with drainage ditches in the existing right-of-way.
Funds for this Amendment will be provided from the 2008 Street Improvements Fund. Adjustments to
current project appropriations will occur as part of the construction contract award such that remaining
funds can be appropriated to other capital roadway needs.
