(Architect) made and entered into City Secretary Contract No. 46791 (The Contract), the 30th day of June, 2015, for Phase 1 site selection services and WHEREAS, further amendment is necessary to reimburse the Architect as part of the design services agreement for additional design services for building programing and conceptual drawings for the new Como Community Center. NOW THEREFORE, City and Architect, acting herein by and through their duly authorized representatives, enter into the following agreement which amends the Contract: 1. Article I — Scope of Services, Paragraph (3) shall be added to read as follows: (3) Provide additional design services for the design and detailing of building programing and conceptual drawings for the new Como Community Center. The cost of this additional service is $48,620.00. The scope of services is described more fully in Perkins+Will letter dated November 17th, 2015, "RE: Additional Services Request for Conceptual Design Services." 2. Article II — Compensation, first paragraph, is amended as follows: "Architect shall be compensated in accordance with the Fee Schedule shown in Attachment "B". Payment shall be considered full compensation for all labor, materials, supplies, and equipment necessary to complete the services described in Attachment "B". However the total fee paid by the City shall not exceed a total of $88,620.00 unless the City and the Architect mutually agree upon a fee amount for additional services and amend this Agreement accordingly." 3. Worth, TX 76102 Re: Additional Services Request for Conceptual Design Services Dear Brian, Perkins+Will is pleased to submit the following Fee Proposal for Architectural Additional Services for the new Como Recreation Center project. In accordance with our June 30, 2015, contract and Exh ibit A dated April 21, 2015 this fee proposal is to provide conceptual design services for a presentation at the upcoming Community Meeting No. 3 on December 10, 2015. The services include: • Preparation of site plan concept diagrams for review with City Staff. • Refinement of 3 preferred site plan concept diagrams • Floor plan diagrams indicating relative program areas and adjacencies • A Power Point Presentation for the Community Meeting including a recap of the previous Community Meetings and new concept diagrams • A follow up meeting with city staff to decide on a preferred concept • Community meeting notes and documentation of the preferred concept The fixed fee for this work is Forty-eight thousand six-hundred twenty Dollars ($48,620) based on the number of staff and total hours required.
