Amendment to City Secretary
Contract No. 28929
THIS contract is made and entered into by and between the City of Fort Worth ("City"), acting and
through Richard Zavala, its duly authorized Interim Assistant City Manager,and NSEAM("Contractor"),
by and through Mr. Lorenzo Dixon, its duly authorized Board Chairman. Contractor's business address is
1150 S. Freeway Room 130,Fort Worth, TX 76104.
WHEREAS, the City of Fort Worth has been notified that it will receive grant monies from the United
States Department of Housing and Urban Development("HUD") through the Community Development
Block Grant ("CDBG") for Year XXIX; and,
WHEREAS,the primary purpose of the CDBG Program is to benefit low and moderate income citizens
of Fort Worth; and,
WHEREAS,the citizens of Fort Worth,the Community Development Council,and the City Council of
Fort Worth have determined that Programs are needed by low and moderate-income citizens of Fort Worth;
WHEREAS, Contract No. 28929 was funded from CDBG Grant Year XXIX and funds remain available
for budgeted amounts to support this contract; and,
WHEREAS, Contract No. 28929 EXPIRED May 31, 2004 and the parties wish to extend that contact
by 90 days to expire on August 31, 2004;
NOW, THEREFORE, THIS AGREEMENT FURTHER WITNESSETH:
THAT, the parties covenant and agree to amend part 1 of Contract 28929 to read as follows:
1. Scope of Services
a. Contractor will administer At Home Block Nurse Program services to citizens of the City of Fort
Worth under the program name NSEAM-Living at Home Block Nurse Program ("Program"), for the
term June 1, 2003 to August 31, 2004.
All other terms and conditions of City Secretary Contract No. 28929 remain in full force and effect.

RECOMMENDATION:
It is recommended that the City Council authorize the City Manager to execute CDBG Year XXIX, ESG
Year 2003-2004 and HOPWA Year 2003-2004 contracts with the following organizations to administer
their respective programs, contingent upon approval and receipt of adequate funding from the U.S.
Department of Housing and Urban Development, for a term beginning June 1, 2003, and ending May
31, 2004:

DISCUSSION:
On April 15, 2003, the City Council approved and adopted the budgets and the appropriation ordinance
increasing estimated receipts and appropriations for the Twenty-Ninth (29th) Year Community
Development Block Grant, (2003-2004) Emergency Shelter Grant (ESG), and (2003-2004) Housing
Opportunities for Persons with AIDS (HOPWA) as part of the City's Consolidated Plan.
The Consolidated Plan, as adopted by the City Council, included program descriptions for each of the
above funded programs.
The above programs are consistent with the City's approved priorities and the City Council's prior
policies and meet the requirements of the Department of Housing and Urban Development regulations
concerning "eligible activities
FISCAL INFORMATION/CERTIFICATION:
The Finance Director certifies that funds will be available in the current operating budget, as
appropriated, of the Grants Fund contingent upon approval and receipt of adequate funding from the
U.S. Department of Housing and Urban Development for the term beginning June 1, 2003, and ending
May 31, 2004.
