AMENDMENT No.01 CONTRACT NO.
TO CITY SECRETARY CONTRACT No. CSC 47981
WHEREAS, the City of Fort Worth (CITY) and CP&Y Inc. , (ENGINEER)
made and entered into City Secretary Contract No. CSC 47981, (the
CONTRACT) which was authorized by M&C C-27760 on the 07 day of June, 2016
in the amount of $ 993, 774 . 00; and
WHEREAS, the CONTRACT involves engineering services for the
following project:
PROJECT NAME: Dewatering Processes at Village Creek Water
Reclamation Facility Project; and
WHEREAS it has become necessary to execute Amendment No. 01 to the
CONTRACT to change the scope of work.

NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT: C

Article I of the CONTRACT is amended to include the revised
engineering services specified in a proposal dated 5/24/2017, a copy of
which is attached hereto and incorporated herein. The cost to City for
the revised scope to be performed by Engineer totals $93, 462. 00, which
will be paid with funds currently existing in the project funds. (See
Attached Funding Breakdown Sheet, Page -3-)

All other provisions of the Contract, which are not expressly
amended herein, shall remain in full force and effect.
EXECUTED and EFFECTIVE as of the date last written by a signatory,

ATTACHMENT A
SCOPE OF WORK
BASIC SERVICES TASK ITEMS
Project Description
The work described in this attachment generally refers to the following engineering activities for
the City of Fort Worth's Village Creek Water Recycling Facility. The following major items are
included in the work.
1. Perform additional data gathering at the Dewatering Facility with the intent to develop
record drawings showing existing equipment and buildings at the facility.
2. Prepare record drawings
3. Prepare mechanical and electrical equipment inventories of major equipment and include
the information in the drawings.
This scope is a net no cost change to the original Dewatering Improvements agreement (CSC
47981). To offset this scope and associated fee, the remaining scope items from the original
Agreement will be suspended until further notice.
SCOPE OF SERVICES
Project Tasks
The following project tasks are included with this amendment.
Task 1: Data Gathering
1. Perform site visits to verify layout and dimensions and to survey the corners of the
buildings and other major structures. During the visit, use a 3D-Lidar scanner to record
the layout of the site and building interiors.
2. Perform site visits to observe electrical gear and document power sources. City staff will
open electrical gear to allow Engineer to observe and document equipment inside the
gear. City staff will be responsible for any lock-out and tag-out required for the work.
3. Perform site visits to gather data on the equipment for use in developing the equipment
inventory. Data collection will include readily available on nameplates or provided by
plant staff. For large equipment, Engineer will contact the vendors to obtain more
detailed information, if necessary.
4. Meet twice with plant staff to review data gathered and obtain additional vendor
information for the equipment inventories.

additional information obtained from Task 1, Engineer will develop the following
drawings.

h) Yard Piping Profiles for gravity lines
i) Road Plan
j) Road Profiles
Task 3: Mechanical Drawings
1. Using information obtained from Task 1 as well as coordination meeting with plant staff,
Engineer will develop the following drawings.
a) PMIDs for the following systems
(i) BFPs and BFP Feed Pumps
(ii) Polymer System
(iii)Chlorine Dioxide System
(iv)Storage Tanks, Valves, and Piping
(v) Lime Feed System
(vi)Storm and Filtrate Pump Systems
b) Dewatering Building Mechanical Plans and 3 Sections
c) Tank Area and Piping Mechanical Plans and 2 Sections
d) Lime System and Dewatered Storage Mechanical Plans and 2 Sections

Task 5: Structural/Architectural Drawings
1. Using information obtained from Task 1, Engineer will develop the following drawings.
a) Dewatering Building Elevations (4 Sides)
b) Floor Elevations.
c) Floor Plan with doors and sizes
d) Layout of Columns in Dewatering Building
Task 6: Maintenance Building
1. Include Maintenance building in the Record Drawings.
a) Floor plan, Mechanical, Architectural, and Electrical elements
Task 7: Fueling Station
1. Include Fueling Station in the Record Drawings
a) Layout, Mechanical, and Electrical elements
Task 8: Contractor's Office
1. Include the contractor's office in the Record Drawings
a) Floor Plan, Architectural, and Electrical elements

I. Include SUE services, three test holes, to locate elevation of buried piping.
a) Perform a Quality Level A (Test Holes) survey of specific locations
b) Three test holes (0' —6' deep in unpaved areas)
c) Survey Data Collection/CAD/PM
Task 10: Structural Condition Report(Optional Item)
1. Visit the site and document current condition of the structures. Field documentation will
include the following.
a) Photographs with Timestamps of the major structural components
b) Note areas of significant cracking in concrete structures
c) Note areas of significant corrosion on concrete and metal components
d) Note structural components that should be monitored due to corrosion or fatigue.
2. Recommendations and probable costs for specific repairs are not included in this work.
3. Deliverable. Develop a Structural Condition Report that includes the following.
a) Overview photographs of the structures
b) Photographs of areas showing significant cracking and corrosion
c) Overall Map indexed to key photographs
d) Text describing the work, findings, and areas recommended for continued
monitoring.
Task 11: Deliverables
1. Engineer will provide the following deliverables for this work.
a) Preliminary Record Drawings. Provide five (5) 11 x 17 copies of the preliminary
drawings for review by City staff. City staff will review the Preliminary Record
Drawings and provide comments. Engineer will meet with the City staff to review
comments and walk through the facility with City staff.
b) Record Drawings. Provide five (5) 11 x 17 copies of the Record Drawings and a PDF
electronic version.

COMPENSATION
Compensation
Engineer will provide services related to the Village Creek Water Reclamation Facility Dewatering
Site Record Drawings for a lump sum fee $93.462.00.
Engineer's compensation for the project will be in accordance with the following schedule.

Prior to advertisement, the contracting/managing department shall determine whether the bid/proposal is one which MBE/SBE
requirements should not be applied. MBE/SBE requirements may be waived upon written approval of the MWBE Manager. If one
