Tarrant County College District -r Tarrant County College District Internship Agency Application & Agreement Community & Industry Education Services Northwest Campus 4801 Marine Creek Parkway Fort Worth, Texas 76179 OFFICIAL RECORD CITY SECRETARY FT.WORTH,TX Internship Agency Agreement This agreement forms the basis of understanding between Tarrant County College District and City of Fort Worth (Name of Agency Sponsor) 1. INTERNSHIP PROGRAM AND OBJECTIVE Establish an internship program with Tarrant County College District(TCCD)to promote and expose career opportunities available within the FORT WORTH WATER DEPARTMENT for Tarrant County College students enrolled in the Wastewater Operator Certificate Program. Students will be able to gain the job experience and assess their interests in pursuing careers in the Wastewater industry. The internship program will also provide a recruitment opportunity for the FORT WORTH WATER DEPARTMENT. Targeted areas for the placement of interns participating in the internship includes Wastewater Treatment. Establish an internship program with Tarrant County College District(TCCD)to promote and expose career opportunities available within the FORT WORTH WATER DEPARTMENT for Tarrant County College students enrolled in the Wastewater Operator Program. Students will be able to gain on the job experience and assess their interest in pursuing careers in the wastewater industry.The internship program will also provide a recruitment opportunity for the FORT WORTH WATER DEPARTMENT. Targeted areas for the placement of interns participating in the internship Wastewater Treatment. 2. STRUCTURE OF INTERNSHIP PROGRAM Each Internship will be for a period of 14 weeks(252 hours). TCCD shall furnish the FORT WORTH WATER DEPARTMENT a list of candidates who have met eligibility requirements. The FORT WORTH WATER DEPARTMENT will select eligible students for an interview based on its needs. TCCD will be notified of students selected to participate in the internship program. The FORT WORTH WATER DEPARTMENT will assign a Mentor from the FORT WORTH WATER DEPARTMENT Staff who is designated to guide the intern through daily tasks. The intern will also interface with the TCCD Program Coordinator.The FORT WORTH WATER DEPARTMENT assigned Mentor will review and sign intern's weekly report and perform intern's evaluation at the end of the internship using TCCD's forms and standards. Internship duties may include: ■ Learning the requirements and regulations regarding the addition of chemicals to drinking water ■ Inspection of the routine operation of equipment ■ Monitoring of operating conditions,meters and gauges ■ Collection and testing of wastewater samples ■ Recording of meter and gauge readings and operational data ■ Cleaning and maintenance of equipment tanks,filter beds and other work areas ■ Familiarity with current U.S. Environmental Protection Agency(EPA)regulations ■ Familiarity with current Texas Commission on Environmental Quality (TCEQ)regulations ■ Familiarity with FORT WORTH WATER DEPARTMENT safety standards and ensuring that such safety standards are met at all times The FORT WORTH WATER DEPARTMENT may propose other aspects of work specifically related to the FORT WORTH WATER DEPARTMENT's targeted area that is not included in the list above and will advise the intern and TCCD of such additional work prior to commencement of the internship. The FORT WORTH WATER DEPARTMENT will provide safety training relevant to the internship position. Interns will attend orientation for the FORT WORTH WATER DEPARMENT'S structure,policies and procedures before commencement of the internship. The internship program will begin in the Summer Semester of 2017. Eligibility Interns must meet the eligibility standards established by TCCD(see the attached). 3. TARRANT COUNTY COLLEGE DISTRICT(TCCD)RESPONSIBILITIES a) Appointing a program coordinator to serve as liaison with the internship sponsor on matters relating to the Internship Program, including the following: Determining if students meet the eligibility requirements for participation in the Internship Program. Recommending students for placement at an appropriate internship site. Assisting students in registering for the correct number of continuing education units(CEUs). Discussing all internship, administrative requirements and deadlines with students. Serving as an on-campus resource should students have questions or concerns during the course of the Internship. Evaluating all internship requirements and assigning the appropriate credit for the work experience. Communicating with sponsor supervisor every 40 hours as a follow-up to status of intern's job performance. b) Working with internship agencies to develop positions which meet the sponsor's staffing needs,as well as personal, educational,and professional needs of interns. c) Identifying and recommending potential interns who meet all qualifications for participation and who can benefit from this type of educational program. d) Maintaining an effective internship program through review of internship assignments and evaluation of interns' performances of the assignments. 4. CITY OF FORT WORTH AGENCY RESPONSIBILITIES a) Appointing an individual to serve as liaison with Wastewater Operator program Coordinator on matters pertaining to this program. b) Providing interns with significant work experiences which contribute to their understanding of and expertise in a major field of study. c) Providing positions in areas commensurate with students' program and career interests. d) Providing the Office of Community&Industry Education(CIE)Services,Northwest Campus with descriptions of internship opportunities. e) Providing on-site supervisor/mentor who is available to the interns and knowledgeable about their role as interns. f) Providing accurate, detailed feedback on the interns' performance on the job, including suggestions for improving or enhancing their performance completing all required forms attached to this Agreement. g) Removing intern immediately from the program if the intern engages in any activity while on City property that adversely affects his/her safety or the safety of the FORT WORTH WATER DEPARTMENT employees. Additionally,the FORT WORTH WATER DEPARTMENT may remove an intern from the internship program, if the conduct of the intern is found to be unsatisfactory by the mentor. h) The City shall compensate each intern at a minimum rate of$11.00 per hour for the time the intern provides FORT WORTH WATER DEPARTMENT the internship duties listed in Section 2. However,both parties agree that the intern is not an employee of the City of Fort Worth. If removed from the program,the intern will be paid for all hours worked prior to the removal. i) Requesting number of interns for the FORT WORTH WATER DEPARTMENT: Location Number interns needed Village Creek Water Reclamation Facility 2 Holly—Field Operations 2 TOTAL 4 Timeline Obtain City Attorney's approval June 2017 Establish responsibility for coordination June 2017 Finalize agreement June 2017 Receive interns July 2017 INSURANCE REQUIREMENTS SECTION A. Prior to the approval of this Agreement by the City, TCCD shall procure,pay for and maintain the following insurance written by companies approved by the State of Texas and acceptable to the City. The insurance shall be evidenced by delivery to the City, at the address shown in REQUIRED PROVISIONS b.(i),certificates of insurance executed by the insurer or its authorized agent stating coverages, limits, expiration dates and compliance with all applicable required provisions.Upon request,the City shall be entitled to receive without expense, copies of the policies and endorsements. The City has no duty to pay or perform under this Agreement until such certificate has been delivered to the City and no officer or employee shall have authority to waive this requirement. SECTION B. The City reserves the right to review the insurance requirements of this section during the effective period of the Agreement and to modify insurance coverages and their limits when deemed necessary and prudent by City's Office of Risk Management based upon economic conditions,recommendation of professional insurance advisors,changes in statutory law,court decisions or other relevant factors.TCCD agrees to make any reasonable request for deletion,revision or modification of particular policy terms,conditions, limitations, or exclusions (except)where policy provisions are established by law or regulation binding upon either party to the Agreement). Upon request,the City and TCCD shall exercise reasonable efforts to accomplish such changes in policy coverages and shall pay the cost thereof. INSURANCE COVERAGE REQUIRED SECTION C. Subject to TCCD's right to maintain reasonable deductibles,TCCD shall obtain and maintain in full force and effect for the duration of this Agreement and any extension hereof, at TCCD's sole expense, insurance coverage in the following type(s)and amounts: 1. Commercial General Liability Insurance including,but not limited to, Premises/Operations,Personal& Advertising Injury,Products/Completed Operations, Independent Contractors and Contractual Liability with minimum combined bodily injury(including death)and property damage limits of$1,000,000 per occurrence, $2,000,000 general aggregate. INDEMNITY To the extent allowed by law,TCCD agrees and is bound to hold the City whole and harmless against any and all claims for damages, costs,and expenses to persons or property that may arise out of, or be occasioned by TCCD's performance under this Agreement or any acts or omissions of its employees under this Agreement;provided, however,that it is not the intention or purpose to create liability against TCCD unless such liability is imposed by law or to waive any legal defenses available to TCCD, including governmental immunity. To the extent allowed by law,the City agrees and is bound to hold TCCD whole and harmless against any and all claims for damages,costs, and expenses to persons or property that may arise out of,or be occasioned by the City's performance under this Agreement;provided,however,that it is not the intention to create liability against the City unless such liability is imposed by law or to waive any legal defenses available to the City. The provisions of this paragraph are solely for the benefit of the parties to this Agreement and are not intended to create or grant any rights,contractual or otherwise to any other person or entity. TERMINATION CLAUSE It is agreed that TCCD or the City may terminate this agreement upon breach of this agreement by the other party. The termination will be effective thirty(30)days from receipt of a letter of notification. Should either party wish to terminate this agreement for any reason other than breach of contract,notification must be given on or before the beginning of the internship period. In such event, any students participating in the internship program will be allowed to complete the program. RELATIONSHIP BETWEEN THE PARTIES Nothing herein shall be construed to create an employer/employee,partnership,joint venture or principal/agent relationship between TCCD and City or between the City's employees and TCCD's employees and interns. Neither party has any express or implied authority to assume any obligation or responsibility on behalf of or in the name of the other party. With independent management of each,they will maintain an effective liaison incorporation. If a dispute arises related to the obligation or performance of either party in this Agreement,representatives of the parties will meet in good faith in attempt to resolve the dispute. GOVERNMENTAL POWERS It is understood that by execution of this Agreement,the City does not,nor does TCCD waive or surrender any of its governmental powers.
