Home My WebLink AboutContract 32301-A5 CITY SECRETARY CONTRACT NO. 145 G� pQJ AMENDMENT No.5 TO CITY SECRETARY CONTRACT No. 32301 WHEREAS, the City of Fort Worth (CITY) and Teague Nall and Perkins, Inc. , (ENGINEER) made and entered into City Secretary Contract No. 32301, (the CONTRACT which was authorized by M&C C-20925 on the 30th day of April, 2005 in the amount of $ 243, 900.00; and WHEREAS, the CONTRACT was subsequently revised by: Amendment Number 1 in the amount of $17,600 .00 which was administratively authorized on January 31, 2007, Amendment Number 2 in the amount of $599, 650.00 which was authorized by M&C #C-23031 on September 9, 2008, Amendment Number 3 in the amount of $123,897.00 which was authorized by M&C #C-25219 on October 18, 2011, and Amendment Number 4 in the amount of $24, 505.00 which was authorized by M&C #C-25896 on October 2, 2012; and WHEREAS, the CONTRACT involves engineering services for the following project: 2004 CIP Stream WF-4 (Lebow Channel) Hydrologic and Hydraulic Analysis, Project No. 00084; and WHEREAS, it has become necessary to execute Amendment No. 5 to the CONTRACT to include an increased scope of work and revised fee. NOW THEREFORE, CITY and ENGINEER, acting herein by and through their duly authorized representatives, enter into the following agreement, which amends the CONTRACT: 1. Article I of the CONTRACT is amended to include the additional engineering services specified in a proposal dated December 6, 2017, a copy of which is attached hereto and incorporated herein. The cost to City for the additional design services to be performed by Engineer totals $8,540. 00. (See Attached Funding Breakdown Sheet, Page -3-) 2 . Article II of the CONTRACT is amended to provide for an increase in the fee to be paid to Engineer for all work and services performed under the Contract, as amended, so that the total fee paid by the City for all work and services shall be an amount of $1,018,092. 00. City of Fort Worth T/PW Storm Water Management Division 900 Monroe Street Fort Worth,Texas 76102 RE:Fee Proposal for Contract Amendment No.5-2004 CIP Stream WF4-Lebow Channel Hydrologic and Hydraulic Analysis City of Fort Worth Project No.00084 (TNP Project No.FTW 15081) Dear Mr.Bennett: Teague Nall and Perkins is submitting this fee amendment for the above referenced contract based on additional services required. Through coordination with the Federal Emergency Management Agency (FEMA) it has been determined they will require the Lebow Channel LOMR be extended approximately 1600'further upstream.This extension is required as a result of revisions to the LOMR tie-in locations as requested by FEMA.The following is a brief description and cost for the additional services to be provided on this project: I. Extension of LOMR Modeling The scope of services necessary to complete the extension of the LOMR is addressed below. A. Design The following additional effort will be required: 1. Incorporate Traulsen warehouse LOMR data (provided by FEMA) into model—this task includes the work necessary to review modeling data from a previous LOMR and incorporate the information into the current Lebow LOMR. 2. Incorporate Transload(Halff)data (incl.structures)into model—this task includes the work necessary to incorporate modeling data from the Transload development located between the existing rail spur and Meacham Boulevard. 3. Floodway Modeling—extend the previously completed floodway models upstream to coincide with the terminus of the new project limits. 4. Revised MT-2 Forms—update the FEMA MT-2 forms to reflect the revised study limits. 5. Revise/Create Topographic Workmap(s)—update and extend the topographic workmaps to reflect the revised study limits. 6. Revised Annotated FIRM Map—revise the Annotated FIRM map to reflect the revised study limits. 7. Revise LOMR Report — revise the LOMR report to incorporate hydraulic modeling assumptions, parameters,and results for the revised study limits. Fee Proposal for Contract Amendment No.4: $8,540 TNP proposes an additional fixed fee of$8,540.00 to accomplish the scope of services described above.This fee shall be full compensation for work required of TNP.
