THIRD AMENDMENT TO
CITY SECRETARY CONTRACT NO. 28004
WHEREAS, on September 23, 2002, the City of Fort Worth("City") and Worth Heights
Neighborhood Association, Inc. ("Contractor")made and entered into City Secretary Contract
No. 28004, (the"contract") as authorized by M&C G-13404 adopted by the City Council on
October 16, 2001;
WHEREAS, the contract provided funding to Contractor pursuant to a grant received by
City from the United States Department of Housing and Urban Development ("HUD") through
the Community Development Block Grant ("CDBG") Program No. B-97-MC-48-0010, Catalog
of Federal Domestic Assistance ("CFDA") No. 14.218, to implement a comprehensive strategy
for revitalization of the Worth Heights neighborhood;
WHEREAS, on October 14, 2004, City and Contractor amended City Secretary Contract
No. 28004 through City Secretary Contract Amendment No. 30717 to extend the effective date
of the contract for six months to provide additional time for Contractor to complete its Model
Blocks objectives;
WHEREAS, on May 16, 2005, City and Contractor amended City Secretary Contract No.
28004 through City Secretary Contract Amendment No. 31829 to extend the effective date of the
contract for an additional six (6)months to provide for additional time for Contractor to complete
its Model Blocks objectives;
WHEREAS, the City Council authorized the reallocation of$50,000.00 in Year XXVIII
CDBG funds from the Worth Heights Small Business Revolving Loan Fund Program and
$20,000.00 in Year XXVIII CDBG funds from the Worth Heights Neighborhood Landscaping
Program to the Worth Heights Minor Home Repair Program in M&C C-20774 adopted on May
31, 2005
WHEREAS, the contract, as amended expires on November 15, 2005 and M&C C-20774
authorized the City Manager to extend or renew the current contract if Contractor requests and
extension or renewal;
WHEREAS, it the mutual desire of City and Contractor to extend City Secretary Contract
No. 28004.
NOW, THEREFORE, City, whose address is 1000 Throckmorton St.,Fort Worth TX
76102, acting by and through Dale A. Fisseler, its duly authorized Assistant City Manager, and
Worth Heights Neighborhood Association, Inc.,whose address is 3724 Jones St., Fort Worth TX
76110 ("Contractor"), acting by and through Raymond Rodriguez, its duly authorized Director,
do hereby agree as follows:

"This contract is extended for one (1) year beginning on November 15, 2005 and
continuing through November 14, 2006."

3. Fundinz This section is amended to be and read as follows:
"City will provide the Contractor with CDBG monies in an amount not to exceed a total of
ONE HUNDRED THIRTEEN THOUSAND FIVE HUNDRED THIRTY EIGHT AND 51/100
DOLLARS ($113,538.51)to be used as follows in the Worth Heights Model Blocks area:
a. Minor Home Repair Program in an amount not to exceed SEVENTY THOUSAND AND
NO/100 DOLLARS ($70,000.00)in CDBG funds.
b. Program Operations in an amount not to exceed FORTY THREE THOUSAND FIVE
HUNDRED THIRTY EIGHT AND 51/100 DOLLARS ($43,538.51)in CDBG funds.
These funds must be expended in accordance with Exhibit "C"—Amended CDBG Operating
Budget. Contractor may not increase or decrease line item amount in its program budget for
program operations without the prior written approval of the City."
III.
Exhibits A,B, and C attached to the contract are hereby replaced with the Exhibits"A",
"B", and"C"attached hereto.
All other terms and conditions of City Secretary Contract No. 28004 not previously
amended or amended herein remain unaffected and in full force and effect, are binding upon the
parties, and are hereby ratified by the parties.

DESCRIPTION:
The Worth Heights Neighborhood Association, Inc. (Contractor) will provide staff, facilities, and
development services to implement the neighborhood revitalization strategy in the areas of housing, and
community development
The period of performance is 12 months from the date of contract execution.

Authorize the City Manager to Extend, Renew or Amend the Current Contract with the Worth
Heights Neighborhood Association, Inc., for the Worth Heights Model Blocks
RECOMMENDATION:
It is recommended that the City Council:
1. Authorize the City Manager to extend or renew the current contract if the Worth Heights Neighborhood
Association, Inc. requests an extension or renewal;
2. Amend the contract if necessary, to achieve Worth Heights' revitalization goals, provided that the
amendment is compatible with the neighborhood revitalization strategy and in compliance with applicable
laws and regulations;
3. Reallocate $50,000 in Year XXVIII CDBG funds from the Worth Heights Small Business Revolving
Loan Fund Program to the new Worth Heights Minor Home Repair Program; and
4. Reallocate $20,000 in Year XXVIII CDBG funds from the Worth Heights Neighborhood Landscaping
Program to the new Worth Heights Minor Home Repair Program.
DISCUSSION:
The Worth Heights Neighborhood Association, Inc. applied for and won the Model Blocks designation in
2001. As they were developing the revitalization strategy for their area, meetings and conversations with
the staff of Accion Texas, led the Worth Heights Model Blocks group to assume that they could contract
with Accion to administer a Small Business Revolving Loan Fund for the benefit of Worth Heights'
residents. However, when Worth Heights was ready to initiate this program, the details of the relationship
could not be worked out. Lacking the City staff and expertise to manage their own program and being
made aware of the availability of funding for small business loans, the Worth Heights Model Blocks
Steering Committee decided to amend their revitalization strategy and request that the funds ($50,000)
previously allocated for the establishment of a Small Business Revolving Loan Fund be reallocated to
establish a Minor Home Repair program for the benefit of Model Blocks residents.
In addition, the Model Blocks community decided that a greater and more lasting benefit could be derived
through this Minor Home Repair program rather than the Neighborhood Landscaping program to which
funds were previously allocated and requests that these funds ($20,000) also be reallocated to the Minor
Home Repair program. These funds will benefit 15 -20 homes in the Model Blocks area.

A Public Comment period from March 14, 2005 to April 18, 2005 was advertised for these actions. No
comments were received.
On April 19, 2005, the above proposal was presented to the Housing and Workforce Development
Committee.
The Worth Heights Model Blocks is located in COUNCIL DISTRICT 9.
FISCAL INFORMATION/CERTIFICATION:
The Finance Director certifies that funds are available in the current operating budget, as appropriated, of
the Grants Fund.
