- FORT WORTH CITY SECRETARY CONTRACT NO. 49557
This Amendment is made between the City of Fort Worth ("FORT WORTH"), a municipal
corporation, acting herein by and through its duly authorized Assistant City Manager and Jeff
Cole Group, LTD ("Consultant"), acting herein through Jeff Cole, its duly authorized President.
WHEREAS, FORT WORTH and Consultant entered into an Agreement identified as
Fort Worth City Secretary Contract No. 49557 ("Agreement") for a period beginning August 4,
2017, and ending September 30, 2018; and
WHEREAS, FORT WORTH desires to amend the Agreement to allow Consultant to
complete additional training for FORT WORTH;
WHEREAS, it is the collective desire of both FORT WORTH and Consultant to amend
the Agreement to increase the total compensation under the Agreement to allow additional
training and to outline the scope of the additional training;
NOW THEREFORE,KNOWN ALL BY THESE PRESENT:
Fort Worth City Secretary Contract No. 49557 is hereby amended to be effective on February 5,
2018 by:
1. Increasing the total compensation included in Section 3 of the Agreement to be a
total amount not to exceed $66,400.00;
2. Add the attached Exhibit A to the existing Exhibit A of the Agreement to outline
the additional Services Consultant will provide and the pricing for the additional
Services. Services will be performed at a specific time upon mutual agreement
between the parties.
All other terms, provisions, conditions, covenants and recitals of said Agreement not expressly
amended herein shall remain in full force and effect.

TRAINING & COSTS:
Lean Yellow Belt Session (February 26-28,2018):
• Session 1 (up to 13 students): ........................................ $13,500
• Students 14-18:................................................................ FREE (2017 unused seats)
• Add'I Students (19-25):.................................................... $750/per student
• Ex: Class of 25: Students 1-13: $13,500+ (7 * $750)+5 Free Students =$18,750 Total
Summit Presentations(March 29-30,2018):
• ($200/Presentation x 6) +travel allowance..................... $2200
TOTAL TRAINING COST NOT TO EXCEED$20,950
PAYMENT TERMS:
Client to be invoiced after completion of each event. Payment terms are Net 30. Please make payment to:

• Pre and post-course videos
• Hard-copy Student binders
• Yellow Belt tools and templates

• (1) instructor including all travel expenses to Fort Worth,Texas
Summit Presentations to Include
• Custom development of presentation and any handouts
• (1) presenter including all travel expenses to Fort Worth,Texas
• E-Copy of the presentation and handouts

Ft.Worth to handle all costs and logistics associated with providing:
• Training facility (preferred layout to be provided by instructor)
• Student parking and security procedures for facility entrance
• Projector and screen
• Flipcharts or whiteboards and markers
• Table and chair for instructor
• Any meals or refreshments for students
• Access to a photocopier
• Point of contact for logistics and receiving shipped student materials
• Student email addresses for pre and post-course surveys &communications
• Any costs associated with ADA compliance for the event
