PERFORMING AGENCY: FORT WORTH DEPARTMENT OF PUBLIC HEALTH
RECEIVING AGENCY PROGRAM: TEXAS DIABETES PROGRAM
TERM: September 01, 2001 THRU: August 31, 2002
SECTION I. SCOPE OF WORK:
PERFORMING AGENCY shall design, implement and evaluate a Diabetes Awareness and
Education in the Community (DAEC) Project. The DAEC project shall target Hispanic
Americans, African Americans, the elderly and other groups at risk for developing Type 2
Diabetes.
PERFORMING AGENCY shall comply with all applicable federal and state laws, rules,
regulations, standards, and guidelines in effect on the beginning date of this contract Attachment
unless amended. PERFORMING AGENCY shall develop the following products:
• An action plan for local enhancement of the DAEC project during the term of this
contract Attachment; and,
• A sustainability plan.
PERFORMING AGENCY is responsible for designing, implementing, and evaluating the DAEC
project. PERFORMING AGENCY shall meet the following objectives through the DAEC
project and shall provide evidence that the DAEC project is meeting the following objectives:
• Implement community activities that have value;
Maintain staffing patterns with qualified staff designated to the DAEC project;
• Reach consensus between PERFORMING AGENCY and the community on community-
wide priorities that describe population-based objectives and achieve compatibility with
the Healthy Communities 2010 Model Standards;
• Increase PERFORMING AGENCY'S credibility within the community, and establish a
"specific efforts and accomplishments" (SEA)performance system;
• Identify and recruit program champions within the organizational structure and the
community;
• Change significant systems in PERFORMING AGENCY'S organizational structure and
its efforts in the community to improve organizational capacity and performance;
• Increase staff development within the organizational structure and in the community;
• Improve PERFORMING AGENCY'S infrastructure and performance;
• Increase access to health care and education services from baseline; and
• Achieve established workload measures by reaching the negotiated number of persons
served through this contract Attachment through awareness activities.

PERFORMING AGENCY shall provide evidence that PERFORMING AGENCY is meeting
end-of-project outcome objectives:
• Increase visibility of the DAEC to at least 15% of the target population; and
• Increase the community's understanding of diabetes by at least 10% from baseline.
All activities shall be performed in accordance with RECEIVING AGENCY'S Program
guidelines as set out in the Request for Proposal (RFP) issued April 27, 2001, and with
PERFORMING AGENCY'S objectives, activities work plan, detailed budget and revisions, if
any, as adopted by RECEIVING AGENCY Program and transmitted in writing to
PERFORMING AGENCY. These documents are incorporated by reference and made a part of
this contract Attachment.
PERFORMING AGENCY shall comply with the following:
• Chapter 103 of the Health and Safety Code;
• "Healthy People 2010". U.S. Department of Health and Human Services, November,
2000;
• RECEIVING AGENCY'S "Quality Care: Client Services Standards for Public Health
and Community Clinics. Texas Department of Health", revised June 1997; and
• "Diabetes Today. A Course on How to Build Skills to Plan and Implement Community
Based Programs for Persons with Diabetes". U.S. Department of Health and Human
Services, December 1993.
Within thirty (30) days of receipt of an amended standard(s) or guideline(s), PERFORMING
AGENCY shall inform RECEIVING AGENCY Program, in writing, if it will not continue
performance under this Attachment in compliance with the amended standard(s) or
guideline(s). RECEIVING AGENCY may terminate the Attachment immediately or within a
reasonable period of time as determined by RECEIVING AGENCY.
PERFORMANCE MEASURES
The following performance measure(s) will be used to assess, in part, PERFORMING
AGENCY'S effectiveness in providing the services described in this contract Attachment,
without waiving the enforceability of any of the other terms of the contract.
Conduct 16 key diabetes education activities
Ensure that at least 500 individuals who have or are at risk for diabetes participate in
key activities
Print a minimum of 0 diabetes related articles
Provide a minimum of 120 diabetes related radio spots
Provide a minimum of 1 television spots

Reach an estimated 2,000,000 people through media exposures
Provide a minimum of 20 community actions
Provide a minimum of 15 community changes
Provide 0 additional or changed objectives as outlined in Application
The numbers above indicate the minimum for each performance measure. This value is subject
to change upon written agreement between RECEIVING AGENCY and PERFORMING
AGENCY.
PERFORMING AGENCY shall submit monthly programmatic reports within fifteen (15) days
after the end of each month that are prepared in the format approved by RECEIVING
AGENCY Program. PERFORMING AGENCY shall also submit quarterly programmatic
reports within fifteen (15) days after the end of each quarter that are prepared in the format
approved by RECEIVING AGENCY Program.
SECTION 11. SPECIAL PROVISIONS:
PERFORMING AGENCY shall include the following credit on all printed, audio or visual
materials developed as part of this program: "Funding provided by the Diabetes Program,
Texas Department of Health."
PERFORMING AGENCY shall immediately notify RECEIVING AGENCY Program
Director, Diabetes Program, of any changes in project activities or staff.

The negotiated indirect cost amount shown above is greater than the maximum allowed by
UGMS and less than the rate pending approval. Payments will be made in accordance with
UGMS until PERFORMING AGENCY files an approved rate with RECEIVING AGENCY.
Indirect charges to this contract may not exceed the amount shown above, except by prior written
approval of RECEIVING AGENCY.

SUBJECT APPROPRIATION ORDINANCE AND GRANT CONTRACT WITH THE TEXAS
DEPARTMENT OF HEALTH AND THE TEXAS DIABETES PROGRAM/COUNCIL FOR
A DIABETES AWARENESS AND EDUCATION IN THE COMMUNITY GRANT FOR
THE FOURTH YEAR FROM SEPTEMBER 1, 2001 TO AUGUST 31, 2002
RECOMMENDATION:
It is recommended that the City Council:
1. Authorize the City Manager to accept and execute a continuation to a grant contract with the Texas
Department of Health (TDH) and Texas Diabetes Program/Council (TDP/C) for $100,000 to be used
to increase diabetes awareness and education at the community level; and
2. Adopt the attached appropriation ordinance increasing appropriations and estimated receipts by
$100,000 in the Grants Fund; and
3. Authorize a non-interest bearing interfund loan from the General Fund to Grants Fund in the amount
of $25,000 for interim financing for this grant pending contract execution to ensure a smooth
transition from one grant funding period to another.
DISCUSSION:
Diabetes is an epidemic and a public health crisis in Texas. The incidence rate has increased ten fold
in the last 60 years (3.7 per 1,000 in 1936 to 30.6 per 1,000 in 1995). The estimated prevalence of
diabetes in Texas is 6.5%. Diabetes impacts minorities disproportionately, i.e., 11.4% for African
Americans and 9.3% for Hispanic Americans. Based on the cost methodology developed by the
University of Texas ("Direct and Indirect Costs of Diabetes in Texas, 1992", by Dr. David Warner, LBJ
School of Public Affairs, et al.), it is estimated that the annual cost of diabetes in Texas is approximately
$4 billion. Using this same cost methodology, the financial burden of diabetes annually for the people
of Fort Worth is estimated to be $102,168,000. Yet we continue to spend health care dollars on
downstream medical technology and treatment, rather than focusing on prevention and intervention.
The medical complications associated with diabetes are serious. It is estimated that as many as 915 to
1,830 Texans will go blind each year as a result of diabetic retinopathy. Another 4,118 Texans will
undergo amputations and an estimated 1,509 Texans will develop kidney disease as a result of
diabetes. Most individuals with Type 2 Diabetes have another chronic condition to manage and each
health condition intensifies the severity of the other condition(s). With diabetes, Cardiovascular
Disease is two to four times more common in individuals, stroke is 2.5 times more common in
individuals, and high blood pressure affects 60 to 65% of the individuals. Over 40% of the individuals
with any chronic health condition have more than one condition to manage, and most chronic health
conditions are behaviorally related. Type 2 Diabetes may be preventable through proper nutrition and
physical activity. It is estimated that 50% of the blindness and 60% of the amputations are preventable.
Even with new medications and management methodology, diabetes is still the leading cause of
blindness, kidney disease, and amputations in Texas, making it one of our most serious, costly, and
common public health threats.

DEPARTMENT OF HEALTH AND THE TEXAS DIABETES PROGRAM/COUNCIL FOR
A DIABETES AWARENESS AND EDUCATION IN THE COMMUNITY GRANT FOR
THE FOURTH YEAR FROM SEPTEMBER 1, 2001 TO AUGUST 31, 2002
The main objective of this Diabetes Awareness and Education in the Community (DAEC) Continuation
Grant is to continue the development and implementation of a community project that reduces the
burden of diabetes, increases the understanding of diabetes, and builds community and organizational
capacity. It is community focused and prevention oriented. The DAEC is modeled after the Healthy
Communities 2000 Model Standards, the Center for Disease Control and Prevention Planned Approach
to Community Health and Diabetes today. The Fort Worth Public Health Department plans to
accomplish this objective by the following:
• Forming regional coalitions in the City with organizations that are interested in increasing
awareness of diabetes prevention and maintenance; each coalition will include a reporting
structure and mechanism; and
• Design and implementation of an awareness campaign which will include media exposure and
grass roots level work; and
• Utilizing existing resources through TDH to evaluate the program.
FISCAL INFORMATION/CERTIFICATION:
The Finance Director certifies that upon execution of the contract and adoption of the attached
appropriation ordinance, funds will be available in the current operating budget, as appropriated, of the
Grants Fund. Upon execution of the grant contract, the interim loan of $25,000 will be returned to the
General Fund.
