AMENDMENT AND RENEWAL AGREEMENT NO. 3
CITY SECRETARY CONTRACT NO.46820
This Amendment(the"Amendment")is made between the City of Fort Worth("City")
and Deaf Link, Inc. ("Consultant"), collectively referred to as the "parties."
WHEREAS,City and Consultant entered into an Agreement identified as Fort Worth
City Secretary Contract ("CSC")No. 46820 with an Effective Date of April 6,2015;
WHEREAS, City and Consultant extended the Agreement through the execution of
CSC No.46820-R2,for a Renewal Term commencing on April 6, 2017, and expiring on April
5, 2018;
WHEREAS, City and Consultant wish to exercise the last renewal, authorized through
execution of CSCO 46820-R1, commencing on April 6, 2018, and expiring on January 31, 2019
to align with the new performance period; and
WHEREAS, the City and Consultant desire to amend the Agreement to increase
annual renewal terms for an additional two, one-year periods beginning February 1, 2019.
NOW THEREFORE, City and Consultant, acting herein by and through their duly
authorized representatives, enter into this Amendment, effective April 6, 2018:
1. Fort Worth CSC No. 46820, as renewed and amended through CSC No.
46820-R 1 and subsequently through CSCO No.46820-R2, is hereby amended by modifying
Section 2 of the Agreement, to add two additional one-year Renewal Terms to the
Agreement. Such Renewal Terms shall be from February 1,2019,through January 31,2020,
and February 1, 2020, through January 31, 2021, and may be exercised at the City's option.
2. The Agreement is hereby renewed for an additional Renewal Term effective
from April 6, 2018, through January 31, 2019, to provide continued services for Accessible
Hazard Alert System AHAS alerts as outlined in the attached Exhibit A, for a 10-month pro-
rated amount of$52,662.00.
3. All other terms, provisions, conditions, covenants and recitals of said
Agreement not expressly amended herein shall remain in full force and effect.

Accessible Hazard Alert System (AHAS"")
Program renewal for the City of Fort Worth,TX
Proposal date: 2-23-2018
Deaf Link proposes renewal of the Accessible Hazard Alert System(AHASTM)including Spanish Language
alerts for the City of Fort Worth.The performance period is for April 6', 2018 through January 31St,2019(10
months). Yearly program costs will be prorated for 10 months. Deaf Link also offers an option to renew the
AHAS program for 2 additional years at current pricing.
Scope of work:
1. Deaf Link will provide the City of Fort Worth Office of Emergency Management with 24/7 capability
to send accessible alerts in response to all Emergency Alert System(EAS)event codes for Warnings,
Watches, and Emergencies,Civil Emergency Messages(CEM), Terrorism events, Amber Alerts, and
National Weather Service watches and warnings.AHAS will send alerts at the direction of the Fort
Worth Office of Emergency Management. AHAS will independently monitor the National Weather
Service and send alerts for Threat to Life and Property watches and warnings for Fort Worth.
2. AHASTM program will send accessible web based alerts in American Sign Language(ASL)video,
English Voice,and Text(maximum length 2 minutes- 150 words). AHAS will also create alerts in
Spanish voice(audio file)with text based translation. All AHAS alerts are sent in email and SMS
formats with associated URL link embedded in the text. AHAS alerts are receivable on all video and
web enabled cell phones, computers,tablets, and refresh braille readers.
Note:ASL video alerts are created in Broadcast quality formats for possible use by local TV stations.
3. Deaf Link will continue to maintain the FTW AHASTM subscriber registration page. This service is
free to the subscriber other than any carrier costs associated with their broadband(phone or internet)
service provider.
4. Deaf Link will continue to host the FTW AHASTM website providing Fort Worth with a web location
where accessible information about AHAS,Active Alerts, FAQ's and Preparedness videos in ASL,
English voice/text can be viewed by the community. AHAS will also maintain a Spanish language
page for where active alerts can be viewed.
5. Community outreach: Deaf Link can work with the City of Fort Worth in hosting community
meetings to encourage personal preparedness and enrollment in the AHAS program. Meeting locations
are determined and paid for by the City,broadband access for demonstrating AHAS is recommended.
Meetings are a scheduled maximum of 2 hours in length. Deaf Link will charge$1500 per meeting
which includes travel and provision of ASL interpreters.
