Q� Acknowledgement of Service Agreement
This Acknowledgement of Service Agreement (this "Agreement") is entered into as of the day of
("Effective Date")by and between AT&T Corn(the"AT&T"),and the CiLy of Ciij of Fort Worth
(the"City").
RECITALS
WHEREAS,the City has issued a Request for Proposal 17-0245 Cable TV, Internet and Dark Fiber Services
WHEREAS, the proposal submitted by AT&T has been selected as the proposal which best meets the needs of the
City for this service;and
WHEREAS, Contractor desires to perform as an independent Contractor to provide DirecTV, DSL and
Internet Services
,upon terms and conditions maintained in this Agreement(and in the attached Exhibits referenced below);
and
NOW THEREFORE,for and in consideration of the mutual promises contained herein,the City and AT&T
agree as follows:
City and AT&T acknowledge the Agreement consists of the following exhibits which are attached hereto and
incorporated herein by reference, listed in their order of priority in the event of inconsistent or contradictory
provisions:

2. Exhibit A—DIR-TEX-AN-NG Communications Technology Services Agreement,DIR-TEX-AN-
NG-CSTA-005("Texas DIR Contract")
3. Exhibit B—AT&T Response to the City of Fort Worth's RFP#17-0245 for Cable TV,Internet and
Dark Fiber Services
IN WITNESS WHEREOF,this Agreement is executed as of the Effective Date.
This Agreement signed by AT&T first, is effective upon Customer signature provided that such fully signed
Agreement is returned to AT&T not more than forty-five (45) days after AT&T's signature date. Any change
made to this document renders the Agreement null and void, except for changes expressly authorized by the
terms of this Agreement.
