WHEREAS, the City of Fort Worth (CITY) and Teague Nall and Perkins,
Inc. , (ENGINEER) made and entered into City Secretary Contract No. 32378,
(the CONTRACT) which was authorized by the City Council by M&C C-20954 on
the 6th day of September, 2005; and subsequently revised by Amendment
Number 1 which was authorized by the City Council on the 12th day of
December, 2006 (M&C C-21883) ;and subsequently revised by Amendment Number
2 which was authorized by the City Council on the 19th day of August,
2008 (M&C C-22981) ; and subsequently revised by Amendment Number 3 which
was authorized by the City Council on the 9th day of June, 2009 (M&C C-
23573) ; and subsequently revised by Amendment Number 4 which was
authorized by the City Council on the 23rd day of March, 2010 (M&C C-
24140) ;
WHEREAS, the Contract involves engineering services for the following
project:
Golden Triangle Boulevard from IH-35W to US 377 ;
WHEREAS, it has become necessary to execute Amendment No. 5 to said
Contract to include an increased scope of work and revised maximum fee;
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the Contract:

Article I of the Contract is amended to include the additional
engineering services specified in the proposal letter dated May 10, 2010,
copies of which are attached hereto and incorporated herein. The cost to
City for the additional design services to be performed by Engineer total
$47,000 .00 .

Article II of the Contract is amended to provide for an increase in
the maximum fee to be paid to Engineer for all work and services
performed under the Contract, as amended, so that the total fee paid by
the City for all work and services shall not exceed the spa.^: of
$2 ,324,206 .00 .

All other provisions of the Contract, which are not expressly amended
herein, shall remain in full force and effect.

Fee Proposal for Contract Amendment No. 5
Dear Mr. Weiss:
Teague Nall and Perkins, Inc. is submitting this fee Amendment for the above referenced contract
based on additional services requested. The following is a brief description and cost for the
additional services to be provided on this project:
I. Realignment of Golden Triangle West of Old Denton Road
The City requests that TNP realign Golden Triangle Boulevard from STA 115+00 to STA
130+00. The roadway is being realigned as a result of an agreement made by the City during
the acquisition of right-of-way from the property located at the southwest corner of Golden
Triangle Boulevard and Old Denton Road.
The realignment of Golden Triangle Boulevard requires revisions to the following plan sheets:
1. Title Sheet
2. Alignment Data Sheets
3. Traffic Control Plans (phases 1-4)
4. Paving & Seeding Layout
5. Golden Triangle Paving Plan & Profile Sheets
6. Old Denton Road Plan & Profile Sheets
7. Intersection Layout Sheet
8. Drainage Area Maps & Computation Sheets
9. Culvert OD1 Plan & Profile
10. Culvert STA 127 Plan & Profile
11. Storm Drain Line "A-1"
12. Storm Drain Line "A-2"
13. Signal Plans at Old Denton Road
14. Signing & Pavement Marking Plans
15. Stormwater Pollution Prevention Plans
16. Plan Quantities & Estimate
11. Stake Right-of-Way for Qwest Communications Utility Relocations
The City requests that TNP stake the proposed right-of-way so that Qwest Communications can
proceed with the relocation of their facilities. TNP will stake 11,500 feet of right-of-way at a
minimum interval of 100 feet.

III. Right-of-Way Documents
The following right-of-way parcels were added or required major revisions:
a. Oncor required an encroachment agreement for the Permanent Slope and
Drainage Easement on the south side of Golden Triangle just east of Alta Vista.
b. Changed Parcel 40 Drainage Easement to a Slope and Drainage Easement to
reduce cost of acquisition.
c. Changed Parcel 51 Drainage Easement to a Slope and Drainage Easement to
reduce cost of acquisition.
d. Revised Parcel 29-A ROW conveyance for realignment of Golden Triangle.
e. Revised Parcel 29-A Utility Easement for realignment of Golden Triangle.
f. Revised Parcel 29 ROW conveyance for realignment of Golden Triangle.
g. Revised Parcel 29 Utility Easement for realignment of Golden Triangle.
h. Revised Parcel 2 ROW conveyance for realignment of Golden Triangle.
Fee Proposal for Contract Amendment No. 5: $ 47,000.00
TNP proposes a fixed fee of$47,000 to accomplish the scope of services described above. This fee
shall be full compensation for work required of TNP and three subconsultants.
Summary of Fees:
I. Golden Triangle Realignment at Old Denton Road $34,000
II. Stake Right-of-Way for Qwest Utility Relocations $7,500
III. Right-of-Way Documents $5,500
Total $47,000
M/WBE Utilization:
TNP commits over 17.4% of the work covered under this amendment to certified M/WBE firms. The
breakdown of this participation is shown below:
