WHEREAS, the City of Fort Worth (CITY) and Teague Nall and Perkins,
Inc. , (ENGINEER) made and entered into City Secretary Contract No. 32378,
(the CONTRACT) which was authorized by the City Council by M&C C-20954 on
the 6th day of September, 2005; and
WHEREAS, the CONTRACT was subsequently revised by: Amendment Number 1
in the amount of $1,711,431 .00 authorized by the City Council on December
12, 2006 (M&C C-21883) , Amendment Number 2 in the amount of $126, 900 . 00
authorized by the City Council on August 19, 2008 (M&C C-22981) ,
Amendment Number 3 in the amount of $32, 600.00 authorized by the City
Council on June 9, 2009 (M&C C-23573) , Amendment Number 4 in the amount
of $51,000 .00 authorized by the City Council on March 23, 2010 (M&C C-
24140) , Amendment Number 5 in the amount of $47,000.00 administratively
authorized on May 26, 2010, Amendment Number 6 in the amount of
$47, 900.00 administratively authorized on February 3, 2011, Amendment
Number 7 in the amount of $17 ,370 .00 authorized by the City Council on
September 20, 2011 (M&C C-25170) , and Amendment Number 8 in the amount of
$21,293 .00 administratively authorized on October 19, 2011; and Amendment
Number 9 in the amount of $22,970 .00 administratively authorized on
November 21, 2011; and
WHEREAS, the CONTRACT involves engineering services for the following
project:
Golden Triangle Boulevard from IH-35W to US 377; and
WHEREAS, it has become necessary to execute Amendment No. 10 to said
CONTRACT to include an increased scope of work and revised maximum fee.
NOW THEREFORE, CITY and ENGINEER, acting herein by and through their
duly authorized representatives, enter into the following agreement,
which amends the CONTRACT:

Article I of the CONTRACT is amended to include the additional
engineering services specified in the proposal letter dated July 27,
2012 , a copy of which is attached hereto and incorporated herein. The
cost to City for the additional design services to be performed by
Engineer totals $42,900 .00.

Article II of the CONTRACT is amended to provide for an increase in
the maximum fee to be paid to Engineer for all work and services
performed under the Contract, as amended, so that the total fee paid by
the City for all work and services shall be the amount of $2,476,639.00 .

All other provisions of the CONTRACT, which are not expressly amended
herein, shall remain in full force and effect.

Fee Proposal for Contract Amendment No.10
Dear Mr. Weiss:
Teague Nall and Perkins, Inc.is submitting this fee Amendment for the above referenced contract
based on additional services requested. The following is a brief description and cost for the
additional services to be provided on this project:
I. Revisions to Traffic Signal Design at Crawford Farms/Ranch Place:
Subsequent to final plan approval,Traffic Operations requested that the traffic signal design be modified
which required a new signal pole layout,adjustments to pedestrian facilities,and revisions to the striping
plan. Effort for this task required a P.E. to redesign sidewalk ramps and crosswalks and a Sr, Traffic
engineer to revise traffic signal layouts for 4 intersections.
Fee: $2,500
If. Revisions to Temporary Signal Design at Old Denton Road:
The Contractor proposed a new traffic control plan affecting the temporary signal at Old Denton Road.
TxDOT has approved the requested traffic control change which requires the temporary signal design to
be modified accordingly. Effort for this task required a Sr.Traffic engineer,P.E.,and Tech to coordinate
design issues with TxDOT, review provided Traffic Control Plan revisions,evaluate compatible signal
phasing against current construction conditions in order to design temporary signal layouts for each
remaining phase of construction.
Fee: $6,170
III. Subsurface Utility Engineering(SUE):
The City requested that TNP perform nine(9)Level A Test Holes,including eight sanitary sewer services
and one test hole on existing gas pipelines. Effort of this task included use of vacuum excavator truck
and crew to locate and then complete Level"A°utility excavation for 9 crossing conflicts with existing
unmarked utility lines. Each utility was surveyed and reported to the City and other Utility companies.
Fee: $11,790
IV. Design of Sanitary Sewer Service Lines:
Eight (8) existing sanitary sewer services were found to be in conflict with the proposed storm drain
improvements TNP performed the design services necessary to relocate the sanitary sewer services,
including development of a special double cleanout detail to fit the project constraints. Effort for this task
required two P.E.'s and Technician to the review and analyze the reported utility surveys, evaluate
alternate solutions, coordinate design options with City of Fort Worth Water Department, and design
custom details for review with City of Fort Worth Water Department.

V. Storm Drain Revision at North Beach Street
During the design phase, SUNOCO had approved the location of the proposed storm drain facilities'
location within their pipeline easement. During construction,they requested that the storm drain facilities
be moved to provide additional separation to their pipeline. TNP revised the plan which involved
changing the paving grades at the intersection to make drainage work.The effort for this task required a
P.E. and Tech to build a custom 3D model of the Roadway surface and retrofit an existing drainage
design to new conditions.
Fee: $4,500
VI. Traffic and Signal Modifications
During construction,the City of Fort Worth's Traffic Department requested the following field changes: 1.
Change ground-boxes to larger size at all locations except at the service location; 2. Provide Count-
down pedestrian heads without audible pushbutton assemblies at all signalized pedestrian crossings;and
3. Provide pedestrian signal heads, pushbuttons, and ground boxes in in Golden Triangle medians
greater than 20'wide(possibly at Old Denton Rd., Crawford Farms Dr./Rancho PI., North Beach St.,
and Alta Vista Rd.); and 4. Retrofit the proposed signal design at N. Beach to accommodate newly
installed signal foundations. Effort for this task will require a Sr.Traffic Engineer,P.E.,and technician to
coordinate design layouts and formats with TxDOT and the City of Fort Worth Traffic Department,
identify existing roadway conditions, and designing proposed changes.
Fee: $9,940

SUBJECT: Authorize Execution of Amendment No. 10 to City Secretary Contract No. 32378, an
Engineering Services Agreement with Teague Nall and Perkins, Inc., in the Amount of
$42,900.00 for Design Revisions on Golden Triangle Boulevard from Interstate 35W to US
Highway 377, for a Revised Total Contract in the Amount of$2,476,639.00 (COUNCIL
DISTRICT 2)
RECOMMENDATION:
It is recommended that the City Council authorize execution of Amendment No. 10 to City Secretary
Contract No. 32378, an Engineering Services Agreement with Teague Nall and Perkins, Inc., in the
amount of$42,900.00 for design revisions on Golden Triangle Boulevard from Interstate 35W to US
Highway 377, for a revised total design contract in the amount of$2,476,639.00.
DISCUSSION:
Golden Triangle Boulevard is currently under construction from Interstate Highway 35W to US
Highway 377 under a contract awarded by the Texas Department of Transportation. The design
contract for this project is between the City and Teague Nall and Perkins, Inc.
This proposed amendment is for revisions to the design contract for permanent traffic signals at Old
Denton Road, Rancho Place/Crawford Farms, North Beach Street, and Alta Vista Road to meet
current City standards, revisions to the temporary traffic signal at Old Denton Road to match current
construction phasing, additional subsurface utility engineering, the design of sanitary sewer lateral
relocations, and revisions to the storm sewer design at North Beach Street. These revisions are
needed in order to address unforeseen conditions encountered on the project during construction and
to meet design, operation and maintenance standards that have been recently updated by the City.
Teague Nall and Perkins, Inc., proposes to complete Amendment No. 10 for a lump sum fee not to
exceed $42,900.00, for a total revised contract amount not to exceed $2,476,639.00. Staff considers
this fee to be fair and reasonable for the scope of services to be performed.
An Engineering Services Contract No. 32378 with Teague Nall and Perkins, Inc., was authorized by
City Council on September 6, 2005. In addition, five previous Contract Amendments were authorized
by Council: M&C C-21883, December 12, 2006, M&C C-22981, August 19, 2008, M&C C-23573,
June 9, 2009, M&C C-24140, April 6, 2010, M&C C-25170, September 20, 2011. Four pervious
Contract Amendments were administratively executed by Staff.
This project is located in COUNCIL DISTRICT 2, Mapsco 21 L, 21 M, 21 R, 22L, 22M, 22N, 22P, 22Q
and 23J.
FISCAL INFORMATION/CERTIFICATION:
The Financial Management Services Director certifies that funds are available in the current capital
budget, as appropriated, of the Street Improvement Fund.
